体重、身高对峰值骨量及腰椎形态的影响

摘    要:目的 通过体重 (W)、身高 (H)对成都地区青壮年峰值骨量及腰椎形态的影响研究 ,为骨质疏松症的诊治中合理应用骨量参数提供科学依据。方法 随机抽取成都地区 2 0~ 39岁 2 37名(男 1 0 8名 ,女 1 2 9名 )排除心、肝、肺、肾、内分泌等疾病 ,骨代谢疾病及脊椎畸形者。应用双能X射线骨密度仪 (DXA)测定全部受试者腰椎 (L1 L4 )正位骨密度 (BMD)、面积 (Area)、骨矿含量 (BMC)及腰椎形态 (横径及高径 )。结果 男性L2 4 BMC、L2 4 Area及L2 4 Area/H均明显高于女性 (P <0 0 5) ,L2 4BMD、L2 4 BMC/W、L2 4 Area/W、L2 4 BMC/H男女无显著性差异 (P >0 0 5)。L2 4 BMD/W、L2 4 BMD/H女性明显高于男性 (P <0 0 5)。由L1 至L4 腰椎的横断面积逐渐增大 ,横径 >高径 ,男性腰椎横径及高径均明显高于女性 (P <0 0 5) ,横径更明显。横径 /W、高径 /H男、女差异无显著性 (P >0 .0 5) ,高径/W女性明显高于男性 ,横径 /H则男性明显高于女性 (P <0 0 5)。结论 L2 4 BMC较L2 4 BMD更好反映体重、身高及骨几何尺寸的变化 ,体重主要影响腰椎横径 ,身高主要影响腰椎高径

Abstract:Objective To research the influence of weight an d height on peak bone mass and lumbar morphosis in the youth in Chengdu, and pro vide scientific basis for use of bone mass parameters in diapnosis of osteoporos is Methods Randomly recruited were 237 subjects (108 males, 1 29 females), aged 20-39 years, in Chengdu, excluding cardiopulmonary disease, h e patic, renal and endocrine diseases, metabolic bone diseases and abnormalities o f spinal column BMD of lumbar vertebrae (L 1-L 4 ), Area BMC and lumbar spine morphosis (width and height) were measuresd by dual energy X-ray absorptiometr y All data were analyzed using SPSS software Results L 2-4 BMC L 2-4 Area/Height and L 2 -4 Area of males were much higher t han those of females (P<0 05) There were no significant differences betwe en L 2-4 BMD, L 2-4 BMC/weight, L 2-4 Area/Weight, L 2-4 BMC /Height of both sexes (P>0 05) L 2-4 BMD/weight and L 2-4 BMD/ height of females were significantly higher than those of males (P<0 05) Cross-sectional area of lumbar spines increased gradually from L 1 to L 4 , an d the width was higher than corresponding height of lumbar spines The width an d height, especially width, of lumbar spines of males were significantly greater than those of females (P<0 05) There were no significant differencese be tween width/weight and height of lumbar spines/height of both sexes (P>0 0 5) Height of lumbar spines/weight of males was much greater than that of f e males, in cortrast to the width/height Conclusion L 2-4 BMC can reflect the change of weight, height and bone morphosis better than L 2-4 BMD does Weight mainly influences on width of lumb ar pine while height mainly heigt of lumbar spines
