Logistic回归分析肝硬化食管胃底静脉曲张破裂出血的相关因素

为了寻找简单易行的方法早期诊断及预防食管胃底曲张静脉破裂出血(EGVB),本文对110例肝硬化按其胃镜检查示曲张静脉直径(D)>5mm、红色征(RC)阳性为分组标准,选择有可能与其有关的9个危险因素进行多因素分析。结果显示年龄、PVD增宽是EGVB的危险因素(P=0．006、0．008),OR值为1．179、4．649,95％CI值为(1．049,1．324),(1．487,14．599),而SVD增宽是EGVB的保护性因素(P=0．036),DR值为0．423,95％CI值为(0．190,0．944)。故PVD增宽、年龄增大是判断肝硬化患者EGVB的重要指标,而SVD增宽是其保护性因素。

Logistic regression analysis on related factors of liver cirrhosis with esophageal - gastric varices bleeding.

To explore the simple and feasible methods of early diagnosis and prevention of esophageal - gastric varices bleeding (EGVB) , the present study was performed to analyze retrospectively the EGVB related factors among 110 patients with liver cirrhosis (LC). Having EGVB risk was defined as esophageal -gastric vasa diameter more than 5mm and red color sign being positive. The risk factors related to EGVB were analyzed using logistic regression analysis. Results dis-plaied age (OR = 1.2, 95% CI 1. 1 - 1. 3, P =0.006) and width of inner diameter of portal ( OR = 4. 7 ,95% CI 1. 5 -14. 6, P = 0. 008) were independent risk factors for EGVB. However, the width of inner diameter of splenetic vein ( OR = 0. 4 , 95% CI 0. 2 -0. 9 , P =0. 036) was protective factor for EGVB. So increased age and inner diameter of portal were independent risk factors, increased inner diameter of splenetic vein was protective factor for prediction of EGVB among patients with LC.
