联合检测脑钠肽和肌钙蛋白I对扩张型心肌病患者心功能的临床意义

摘    要:目的探讨联合检测脑钠肽(BNP)和肌钙蛋白I(cTnI)对扩张型心肌病(DCM)患者心功能的临床意义。方法选择2012年1月-2013年6月我院心内科和急诊科入住的DCM患者76例,将其按照心功能分级分为心功能Ⅱ级组22例,心功能Ⅲ级组26例,心功能Ⅳ级组28例;选择同期我院体检健康者39例作为正常对照组。检测受试者血清BNP、cTnl水平及左室射血分数(LVEF)。结果心功能Ⅳ级组BNP、cTnI水平〉心功能Ⅲ级组〉心功能Ⅱ级组〉正常对照组(P〈0.05)。DCM患者血清BNP与cTnl呈正相关(r=0.775,P〈0.05),血清BNP和cTnI与LVEF呈负相关(r值分别为-0.430、-0.542,P〈0.05)。结论血清BNP水平对评价DCM患者心功能损害的严重程度具有重要的临床意义,与cTnI联合检测可预测心力衰竭的严重程度。

Abstract:Objective To investigate the clinical significance of combined detection of serum brain natriuretic peptide (BNP) and Troponin I (cTnI) on cardiac function in patients with dilated cardiomyopathy (DCM). Methods 76 patients with DCM in Department of Cardiology and Emergency in our hospital were choose from January 2012 to June 2013, they were di- vided heart function lI group of 22 cases, heart function m group of 26 cases, heart function IV group of 28 cases according to NYHA; 39 healthy cases were chosen as normal control group during same period in our hospital. The serum BNP, cTnI levels and left ventricular ejection fraction (LVEF) were detected. Results The serum BNP and cTnI levels in heart function IV group 〉 heart function m group 〉 heart function II group 〉 normal control group (P 〈 0. 05 ). In the DCM patients, serum BNP was positively correlated with serum cTnI ( r = 0. 775 ), serum BNP and cTnI levels were negatively correlated with LVEF ( r = - 0. 430, - 0. 542 ; P 〈 0. 05). Conclusion The serum BNP level in DCM patients has important clinical significance on the evaluation of damage severity of cardiac function, and combined detection of serum BNP and cTnI can predict the severity of heart failure.
