甲状腺癌不同范围切除后甲状腺功能的变化

目的探讨分析甲状腺癌不同范围切除术后甲状腺功能的变化。方法回顾性分析290例甲状腺癌患者的临床资料，根据手术方式将所有患者分为全切组次全切除组各145例，于手术前后测定所有患者血清甲状腺激素的水平。结果所有患者T3与FT3水平较术前明显下降（P〈0．05），术后1d，所有患者T4、FT4水平较术前有所上升，术后3d，所有患者T4、FT4水平要显著低于手术前（P〈0．05），但TSH水平要明显高于术前（P〈0．05）。结论甲状腺癌腺叶全切术与腺叶次全切除术后患者的甲状腺功能并无明显差异。术后3d患者的甲状腺功能出现较为明显下降．此时应及时补充甲状腺激素。

Changes in thyroid function in different range of thyroid cancer resection

Objective To study the changes in thyroid function in different range of thyroid cancer resection. Methods A total of 290 cases of thyroid cancer were retrospectively analyzed. 145 patients who accepted total thyroidectomy were the total resection group,while the other 145 patients who accepted subtotal thyroidectomy werethe subtotal re- section group. The serum thyroid hormone were measured after the surgery. Results After the surgery, level of T3 and FT3 significantly decreased compared to that of the preoperative （P 〈 0.05） ; 1 day after surgery, T4, FT4 levels of all the patients increased eompaed to that before the surgery. 3 days after surgery, T4, FT4 levels of all the patients were significantly lower than that before ssurgery （P 〈 0.05）, but TSH level was significantly higher than that before the surgery （P 〈 0.05）. Conclusion Thyroid function of patients with thyroid cancer is not obviously different no matter they ac- cept the total thyroidectomy or the subtotal thyroidectomy. 3 days after the surgery, patient＇s thyroid function declines significantly and thyroid hormone should be replenished then.
