血与尿甘氨酰脯氨酸二肽氨基肽酶比值在原发性肝癌诊断中的意义

目的 探讨血清甘氨酰脯氨酸二肽氨基肽酶（GPDA）水平及血与尿GPDA比值在原发性肝癌诊断中的临床意义。方法 测定15例原发性肝癌、32例急性肝炎、15例慢性活动性肝炎、11例肝硬化等肝脏疾病患者的血与尿的GPDA活性和甲胎蛋白（AFP），计算出血与尿GPDA比值并进行统计分析。结果 原发性肝癌患者的血与尿GPDA比值明显高于对照组，诊断原发性肝癌时，以单项指标分析血与尿GPDA比值与AFP有相似的灵敏度（分别为80．0％和86．7％），二者均较血清GPDA活性（66．7％）高；如2项指标联合检测，可使敏感度提高到93．3％。结论 血与尿GPDA比值与肿瘤标志物AFP联合检测对原发性肝癌的诊断有较好的应用价值。

Diagnostic value of the ratio of serum to urine content of glycylproline dipeptidyl aminopeptidase (GPDA) in primary hepatic carcinoma

Objective To explore the diagnostic value of the ratio of serum to urine content of glycylproline dipeptidyl aminopeptidase （GPDA） in primary hepatic cancer. Methods Serum GPDA, urine GPDA and alpha fetoprotein （AFP） contents were measured for 15 cases with primary hepatocellular cancer, 32 cases with acute hepatitis, 15 cases with chronic active hepatitis and 11 cases with hepatic cirrhosis. The ratio of serum GPDA to urine GPDA was calculated, and then statistical analysis was carried out. Results The ratio of serum GPDA to urine GPDA in primary hepatocellular cancer group was significantly higher than that of healthy control group. The sensitivity of the ratio of serum GPDA to urine GPDA （80.0%） or AFP （86.7%） was much higher than that of serum GPDA activity （66.7%）. Combined detection of two indices could raise the sensitivity to 93.3%. Conclusion Combined detection of two indices （ratio of serum GPDA to urine GPDA, AFP） contributes to the diagnosis of primary hepatic carcinoma
