大肠癌端粒酶活性、p53蛋白表达的联合检测及其临床意义

摘    要:目的 :探讨端粒酶活性、p5 3蛋白基因在大肠癌发生发展中的作用 ,以及端粒酶与临床病理特征及 p5 3蛋白之间的关系。方法 :TRAP法检测 4 0例大肠癌及癌旁正常组织中端粒酶活性 ,采用免疫组织化学法检测 4 0例大肠癌组织及癌旁组织中 p5 3蛋白的表达 ,并对端粒酶活性与临床病理特征以及 p5 3蛋白表达的关系进行分析。结果 :大肠癌组织端粒酶活性、p5 3蛋白阳性率分别为 82 .5 % (33/ 4 0 )、72 .5 % (2 9/ 4 0 ) ;癌旁正常组织端粒酶活性、p5 3蛋白阳性率分别为 12 .5 % (5 / 4 0 )、7.5 % (3/ 4 0 ) ,大肠癌组织显著高于正常组织 ,P <0 .0 1;大肠癌组织端粒酶活性与临床病理特征 (包括病理分化程度和是否有淋巴结转移 )之间无显著相关性 ;端粒酶活性与 p5 3蛋白表达之间无明显相关性。结论 :端粒酶在大肠癌组织高表达和在癌旁正常组织低表达的特性 ,有可能使端粒酶成为大肠癌诊断的一种标志物 ;p5 3参与大肠癌的发生发展过程 ;端粒酶活性与 p5 3蛋白表达之间无明显相关性 ,提示大肠癌是由多基因参与的疾病。

Abstract:Objective:To study the role of telomerase activity, p53 in pathogenesis and progression of colorectal cancer specimens and the relation among them. Methods:Telomerase activity levels were measured in 40 colorectal cancer and adjacent normal tissue by a modified telomeric repeat amplification protocol, and p53 mutations were identified by immunohistochemical staining. Results were compared with clinical pathological features. Results:Telomerase activity was present in 33 of 40(82.5%) tumors and in only 5((12.5%) of normal specimens( P < 0.01 ). The p53 mutations were present in 72.5% of colorectal cancer. No significant correlation between histopathologic grading of the tumor, lymphatic transfer, p53 and telomerase activity levels was found. Conclusions: Telomerase presents high expression in colorectal cancer and low expression in normal colorectal tissue. It suggests that telomerase may be a useful diagnostic marker of colorectal cancer;there is no significant correlation between telomerase and p53. Multiple genes may be involved in the pathogenesis of colorectal cancer.
