金黄色葡萄球菌抗生素耐药相关基因检测及蛋白A基因多态性分型研究

摘    要:目的 检测2011-2012年深圳市南山区某医院及环境样本中金黄色葡萄球菌(staphylococcus aureus,SA)耐药基因,对SA进行蛋白A基因多态性分型(single locus DNA-sequencing of the repeat region of the Staphylococcus protein Agene,Spa)研究.了解SA耐药基因分布情况以及Spa分型特点.方法 采用PCR技术检测SA的12种耐药基因,对SA蛋白A基因进行扩增,PCR产物测序,序列上传数据库进行比对.结果 27株菌分离自院内病人样本,13株检出mecA基因.其中15株菌检出2种以上耐药基因,3株菌未检出任何耐药基因.16株菌分离自环境样本,未检出mecA基因.15株菌检出ant(4',4")基因,仅1株菌检出Aac(6')/aph(2")基因,其余耐药基因未检出.43株SA分为15种Spa型,t091型为优势型别,检出率为27.9%(12/43),此型别在所有类型的样本中均有检出.结论 院内样本中,不同样本分离菌株携带耐药基因存在差异,院内病人样本分离菌株呈多重耐药.Spa分型,利于深圳市SA分子分型数据库的建立,对于疾病溯源有着重要意义.

Abstract:Objective To detect the antibiotic-resistant genes of Staphylococcus aureus (SA) in hospital and environment's samples,and research the single locus DNA-sequencing of the repeat region of the Staphylococcus protein A (Spa) gene of 2011-2012 in Nanshan district of Shenzhen and to know the antibiotic genes distribution and Spa typing in SA.Methods The 12 kinds antibiotic genes and the repeat region of the A gene were detected by polymerase chain reaction(PCR).Ampli fied products of (Spa) A gene were sequenced and the results were compared with the Spa database.Results 13 strains carried mecA genes in 27 strains SA from samples of patients.The 15 strains carried more than 2 kinds antibiotic-resistant genes and there were no any antibiotic-resistant genes were detected in three strains.The mecA gene were not detected in environment's samples.Aac (6')/aph (2'') gene was detected in only one strain,ant (4',4'') gene were detected in 15 strains,the others antibiotic-resistant genes were not detected.There were 15 Spa typings in 43 strains,the preponderant Spa typing was t091,the detection rate was 27.9% (12/43),and the typing was exist in all Spa typing.Conclusion There were some difference of carring antibiotic-resistant genes between different samples from patients,the strains from patients had existed multiple resistant.To detect the antibiotic-resistant genes distribution can guide clinical Medicalion,control infection of hos pital and moniter bacteria resistant of environment.There were some important significance in disease traceability for building Spa typing database of SA in Shenzhen.
