骨质疏松症患者骨代谢生化指标检测的临床意义

摘    要:目的 探讨骨质疏松症(osteoporosis,OP)患者骨代谢生化指标检测的临床意义.方法 选取50例骨质疏松症患者(OP组)及50例同期健康体检者(对照组),应用电化学发光免疫分析法分别检测两组钙素(N MID),Ⅰ型胶原氨基末端延长肽(PINP),β胶原特殊序列(β cTX),并比较N MID,PINP和β cTX三者联合检测的阳性率与各指标单项检测的阳性率.结果 OP组的N MID,PINP和β cTX分别明显高于正常对照组(66.51±2.37 ng/ml,63.92±4.35 ng/ml,1.02±0.14 ng/ml vs 12.64±4.65 ng/ml,21.08±8.17 ng/ml,0.31±0.08 ng/ml),差异有统计学显著性意义(t=5.863,P<0.05;t=16.273,P<0.01; t=3.713; P<0.05).OP组绝经后女性的N MID,PINP和β cTX均明显高于绝经后女性正常对照组(55.06±1.21 ng/ml,45.28±2.16 ng/ml,0.96±0.13 ng/ml vs 34.92±6.73 ng/ml,26.01±3.25 ng/ml,0.56±0.19 ng/ml),差异有统计学显著性意义(t=5.163,P<0.01;t=7.935,P<0.01;t=2.823,P<0.05).N MID,PINP和βcTX三项骨代谢指标联合检测的阳性率均明显高于N MID,PINP,β cTX各单项指标检测的阳性率(χ^2=4.863,P<0.05;χ^2=4.732,P<0.05;χ^2=4.339,P<0.05;x=4.278,P<0.05;χ^2=3.983,P<0.05).结论 N MID,PINP,β-cTX是反映骨形成和骨吸收特异度和敏感度较高的指标,将上述指标联合检测有利于对OP的诊断、预防及治疗.

Clinical Biochemical Markers of Bone Metabolism in Patients with Osteoporosis

Abstract:Objective To investigate the clinical significance of detecting biochemical markers of bone metabolism in patients with osteoporosis.Methods Selected 50 cases of patients with osteoporosis (OP group)and 50 healthy people over the same period (control group) as objects,and the level of N MID,PINP and β cTX were detected,and the positive rate of N MID,PINP and β cTX combined detection were compared with each individual test positive indicators by application of statistical methods.Results The level of N MID,PINP and β cTX of OP group was significantly higher than the normal control group (66.51±2.37 ng/ml,63.92±4.35 ng/ml,1.02±0.14 ng/ml vs 12.64±4.65 ng/ml,21.08±8.17 ng/ml,0.31±0.08 ng/ ml),the difference was significant (t=5.863,P〈0.05; t=16.273,P〈0.01; t=3.713,P〈0.05).The level of N-MID,PINP and β cTX of postmenopausal women of OP group was significantly higher than the normal control group (55.06 ± 1.21 ng/ml,45.28±2.16 ng/ml,0.96±0.13 ng/ml vs 34.92±6.73 ng/ml,26.01± 3.25 ng/ml,0.56±0.19 ng/ml) and the difference was significant (t=5.163,P〈0.01 ; t=7.935,P〈0.01 ; t=2.823,P〈0.05).Bone metabolism markers posi tive rates of N MID,PINP and β cTX were significantly higher than the single detection,the difference was significant (χ^2 =4.863,P〈0.05;χ^2=4.732,P〈0.05;χ^2=4.339,P〈0.05;χ^2=4.278,P〈0.05;χ^2=3.983,P〈0.05).Conclusion NMID,PINP and β cTX is a reflection of bone formation and resorption index,has higher specificity and sensitivity in measur ing bone mineral density,at the same time joint detection of these indicators contribute to diagnosis,prevention and treatment for the patients of osteoporosis.
