慢性胃炎患者腻苔病理及细胞化学成分研究

摘    要:目的:检测慢性胃炎患者不同舌苔不同化学成分的变化,尝试为临床检测提供一种具有中医特色的指标。方法:选择慢性胃炎患者舌苔为研究对象,采用超声破碎法制成细胞匀浆,试剂盒检测,用UV-756MC型分光光度计检测舌苔细胞匀浆中碱性磷酸酶(ACP)、乳酸脱氢酶(LDH)、琥珀酸脱氢酶(SDH)、糖原(PAS)的含量,对照组选择正常人正常淡红舌,薄白润苔。结果:慢性胃炎组LDH含量较正常组升高(P<0.01),PAS含量较正常组降低(P<0.01);慢性胃炎腻苔组舌苔细胞SDH含量较非腻苔组升高(P<0.05);腻苔组、非腻苔组LDH含量较正常组升高(P<0.01),而PAS含量较正常组降低(P<0.01)。慢性胃炎腻苔组和非腻苔组的胃黏膜病理学指标间未见显著性差异(P>0.05)。结论:慢性胃炎腻苔的形成可能是舌苔上皮细胞能量代谢平衡被打破,细胞角化增值的速度大于凋亡的速度,上皮细胞密度增加,镶嵌于舌状乳头的角化树间,不易脱落,最终使舌苔呈油腻状紧贴于舌面。

Abstract:The study was aimed to test different chemical composition changes in tongue coatings of chronic gastritis cases in order to provide clinical indicators with Chinese medicine characteristics. Tongue coatings of chronic gastritis cases were used as research objects. Tongue coating samples were taken and made into cell suspension liquid by ultrasonic broken and Elisa testing. The homogenate was tested by the UV spectrophotometer detection 756MC -type for the content of alkaline phosphatase (ACP), lactate dehydrogenase (LDH), butanedioic acid dehydrogenase (SDH), and glycogen (PAS) of tongue coating. The tongue coating of normal individual, which is "pale-red tongue with thin and white coating", was selected as control group. The results showed that the LDH content of chronic gastritis group was higher than the normal group (P< 0.01). But the PAS content was lower than the normal group (P< 0.01). The SDH content of greasy coating in chronic gastritis cases was higher than that of non-greasy coating (P< 0.05). The LDH content of both greasy coating group and non-greasy coating group was higher than the normal group (P< 0.01). But the content of PAS was lower than the normal group (P< 0.01). There was a significant difference in the aspect of histopathology (P> 0.05) between the chronic gastritis with greasy coating group and nongreasy coating group. It was concluded that the forming of greasy coating in chronic gastritis cases might be caused by the loss of energy balance in the epithelial cell metabolism. The speed of cell cutinization and increasing was faster than the speed of cell apoptosis, leading to the density of epithelial cells embed in the cutinization trees of the tongue papilla. And a greasy coating was formed on the surface of the tongue at last.
