摘    要:目的研究周围型肺癌增殖细胞核抗原(PcNA)表达与CT表现的关系。方法运用SP免疫组化法,检测63例经病理证实的周围型肺癌组织中PCNA的表达,并回顾性分析其与术前CT征象间的关系。结果PCNA表达与瘤体大小、深分叶征、空洞、胸膜凹陷征、纵隔淋巴结转移有关,与毛刺征无关。结论周围型肺癌PCNA表达与CT征象的相关性研究,可为阐明周围型肺癌CT表现的分子机理及临床评价和治疗周围型肺癌提供有价值信息

Abstract:Objective] To investigate how Proliferating cell nuclear antigen (PCNA) expression relates to computed tomography (CT) findings in peripheral lung cancers. Methods] 63 paraffin-embedded specimens from patients with pathologically verified peripheral lung cancer who underwent surgical resection in our university hospital from 1991 to 2001 were studied retrospectively. The expression of PCNA was investigated by immunohistochemistry.Preoperative CT images of each patient were obtained and reviewed independently by three observers who didn′t know the immunohistochemical staining results. Correlation between the immunological findings and CT features was investigated in a lesion-by-lesion fashion. Chi-square test was used for statistical analysis. Results] PCNA grades Ⅲ~Ⅳ were found in 30 (47.6%) of the 63 cases. No association was found between PCNA expression and histological type, whereas a significant correlation of PCNA expression to histological grade was observed. PCNA grades Ⅲ~ Ⅳ appeared more frequently in poorly differentiated tumors (68.4%) than in well differentiated tumors (31.3%) or moderately differentiated tumors (36.4%) (P <0.05). PCNA expression correlated with tumor size, lobulated border,cavitation, pleural indentation and mediastinal lymph node enlargement, but not with spiculation. Conclusion] The relationship of CT features of peripheral lung cancers with PCNA expression allows better understanding of the molecular basis of CT signs, and may have implications for clinical management and treatment evaluation of these neoplasms.
