摘    要:目的探讨在有心肌收缩及舒张功能障碍的患者中,评估超声心动图参数与血浆NT-proBNP水平的关系。方法该项研究包括96例心力衰竭患者。他们同时接受超声心动图及NT-proBNP检测。结果发现NT-proBNP水平与以下几个超声心动图参数之间有显著相关性,二尖瓣环舒张晚期速度(Am)(r=-0.747,P=0.000),二尖瓣环收缩期运动峰值速度(Sm)(r=-0.731,P=0.000),二尖瓣环舒张早期运动速度(Em)(r=-0.508,P=0.000),二尖瓣口舒张早期血流速度与二尖瓣环舒张早期运动速度之比(E/Em)(r=0.532,P=0.000),左室射血分数(LVEF)(r=-0.819,P=0.000)。多元回归分析显示,logNT-proBNP水平与年龄、左室射血分数、二尖瓣环舒张晚期速度及二尖瓣环舒张早期运动速度独立相关(R2=0.78,P=0.0001),与左室射血分数的相关性最强(β=-0.55)。结论 NT-proBNP水平与超声心动图参数具有相关性,并且是收缩性及舒张性心力衰竭简单而精确的标志物。

Predictive value of NT-proBNP on left ventricular contraction and diastolic dysfunction

Abstract:Objective To explore the correlation between echocardiographic parameters and plasma N-terminal pro brain natriuretic peptide (NT-proBNP) level in patient with systolic or diastolic dysfunction. Methods The study included 96 heart failure patients. They underwent conventional and tissue Doppler imaging (TDI) echocardiography and NT-proBNP level was measured at the same time. Results Significant correlations were found between NT-proBNP level and late diastolic mitral annulus velocity (Am) (r = -0.747, P = 0.000), systolic mitral annulus velocity (Sm) (r = -0.731, P = 0.000), early diastolic mitral annulus velocity (Em) (r = -0.508, P = 0.000), early transmitral to Em velocity ratio (r = 0.532, P = 0.000), LV ejection fraction (r = -0.819, P = 0.000). In multiple regression, log NT-proBNP levels were independently related to age, LV ejection fraction, Am velocity and Em velocity (R2 = 0.78, P = 0.000 1), the relation to ejection fraction was the strongest (β = -0.55). Conclusion NT-proBNP levels correlate with echocardiographic parameters, and are simple, accurate markers of systolic and diastolic heart failure.
