胸腔镜下肺减容术治疗肺气肿临床效果分析

摘    要:目的探讨胸腔镜下肺减容术治疗肺气肿的临床效果。方法总结26例胸外科收治的严重肺气肿患者实施胸腔镜下肺减容术的经验,比较治疗前后患者肺功能变化情况,并总结患者手术时间、术后使用呼吸机时间、术后住院时间以及随访3个月发生的并发症。结果治疗后患者FVC、FVC%以及FVC1.0均显著优于治疗前,手术时间65~240min,平均120min,使用呼吸机时间为5~48h,平均16.3h,术后住院时间20~90d,平均39.5d,术后3个月死亡1例,严重肺心病4例,生活质量明显改善者21例。结论胸腔镜微创下实施肺减容术治疗肺气肿是一种有效的临床治疗方法,可以显著改善患者近期肺功能,提高患者生活质量,值得临床重视。

Abstract:Objective To investigate the clinical effect of lung volume reduction surgery in the treatment of emphysema with thoracoscopic.Methods The experience of pulmonary emphysema with thoracoscopic for treating 26 cases with severe emphysema in our chest surgery department were summarized,and changes in lung function before and after treatment were compared,the time of the operation,using breathing machine and hospitalization in postoperative and the complications occurred after follow-up for 3 months were summarized.Results After treatment,FVC,FVC%and FVC1.0 of patients were significantly better than before,the time of the operation was 65-240 min,with an average of 120 min,the time of using breathing machine was 5-48 h,with an average of 16.3 h,the time of hospitalization in postoperative was 20-90 d,with an average of 39.5 d,1 case died in postoperative,4 cases were found with severe pulmonary heart disease,the life quality of 21 cases were improved markedly.Conclusion LVRS with minimally invasive thoracoscopic in the treatment of pulmonary emphysema is an effective clinical treatment method,it can significantly improve recent lung function and life quality of patients,so it is worthy of attention in clinic.
