甲状腺微小乳头状癌的超声诊断及临床价值分析

目的探讨甲状腺微小乳头状癌的超声诊断意义及分析其临床价值。方法应用彩色多普勒高频超声,与病理诊断结果对照分析甲状腺肿瘤患者48例。结果直径≤1cm的甲状腺微小乳头状癌14例,其他类型如甲状腺腺瘤34例。经过高频彩色多普勒超声高分辨地显示出肿瘤的外部形态及其内部回声情况,两组结节的包膜、周围声晕、后方衰减及其内部微小钙化差异均有统计学意义（P〈0.05）。结论边缘粗糙毛刺状、形态不规则、结节内部存在微小钙化灶等3个重要特征,是甲状腺微小乳头状癌诊断的重要指标,彩色多普勒高频超声对鉴别甲状腺微小乳头癌与其他类型肿瘤具有很高的临床价值。

Ultrasound diagnosis and clinical value analysis of the small thyroid papillary carcinoma

Objective To explore the ultrasound diagnosis and clinical value analysis of the small thyroid papillary carcinoma. Methods The ultrasonographic charactertics of 48 cases of patients were analyzed compared with the result of pathology. Results Fourteen cases of the diameter of ≤1 cm small thyroid papillary carcinoma, other types, such as thyroid adenoma 34 cases. The colorful high frequency ultrasound could show clearly the the PTMC were irregular shape, absent halo sign, ill defined, hypoechoic, solid, heterogeneous, microcalcifications, internal blood flow pattern, metastatic lymph nodes. The envelope of the two groups of nodules, the surrounding sound halo behind theattenuation of internal microcalcifications differences were statistically significant (P < 0.05). Conclusion Edge of the rough burr-like, irregular, nodular exist within the three important characteristics of the micro-calcifications, etc. is an important indicator of small papillarycarcinoma of the thyroid. Colorful high frequency ultrasound is very effective method to identify the papillry thyroid microcarcinoma and the other carcinoma of papllry, and its clinical meaning is so important.
