髓芯减压及异体螺纹骨笼结合自体骨植入治疗早期股骨头坏死21例疗效分析

摘    要:目的回顾性分析髓芯减压及异体骨笼结合自体骨植入治疗早期股骨头坏死的临床疗效,同时观察股骨头坏死灶的修复及髋关节功能改善情况。方法 2008年9月~2010年12月选取早期股骨头坏死患者21例(24髋);男13例,女8例,年龄25~48岁,其中ARCO一期8例10髋,二期10例11髋,三期3例3髋,采用髓芯减压及异体螺纹骨笼结合自体骨植入治疗。结果经2年随访定期行X线检查,股骨头坏死体积由32.15%降至11.28%。患者术前的平均Harris评分为61.2分,术后平均评分为86.7分,平均提高了25.5分,患侧髋关节功能明显改善。结论髓芯减压及异体螺纹骨笼结合自体骨植入治疗早期股骨头坏死安全有效,有利于骨坏死的修复与重建,是一种理想的治疗方案。

Abstract:Objective To retrospectively analyze the clinical efficacy of core decompression and allograft threaded bone cage combined with autologous bone implant on treatment of early femoral head necrosis, while observe the repair of femoral head necrosis and hip function improvement. Methods Twenty one cases (24 hips) of early femoral head necrosis patients from September 2008 to December 2010 were selected, 13 cases of males and 8 cases of females, age 25-48 years old, among whom 8 cases 10 hips were the ARCO first period, 10 cases 11 hips were the second period, 3 cases 3 hips were the third period, all patients were given core decompression and allograft threaded bone cage combined with autologous bone implant. Results After two years of follow-up of regular X-ray examination, femoral head necrosis volume decreased to 11.28% by 32.15% . The preoperative mean Harris hip score was 61.2 points, the mean postoperative score was 86.7 points, an average of 25.5 points, ipsilateral hip function improved significantly. Conclusion Core decompression and allograft threaded fusion cage combined with autogenous bone implant treatment of early femoral head necrosis is safe and security, is conducive to the restoration and reconstruction of bone necrosis, is an ideal treatment plan.
