外固定联合经皮骶髂螺钉治疗Tile C型骨盆骨折疗效分析

目的探讨外固定联合经皮骶髂螺钉治疗TileC型骨盆骨折的疗效。方法回顾性分析经外固定联合经皮骶髂螺钉治疗（研究组,30例）及单纯采用外固定治疗（对照组,33例）的TileC型骨盆骨折患者的治疗结果,比较两组患者术后Matta评分及Majeed功能评分情况,并观察两组患者不良反应情况。结果研究组30例患者均一期愈合;对照组一期愈合27例,二期愈合6例。研究组患者术后Matta评分及Majeed功能评分优良率（90.0%、90.0%）均显著高于对照组（75.8%、72.7%）,差异有统计学意义（χ2=7.64、6.08,P〈0.05）。研究组无一例发生不良反应;对照组8例出现复位偏移,后改为外固定联合经皮骶髂螺钉治疗治愈。结论外固定联合经皮骶髂螺钉治疗TileC型骨盆骨折手术损伤小,安全有效,无明显不良反应,值得临床推广应用。

Analysis of the therapeutic effect of percutaneous iliosacral screws combined with external fixation in the treatment of Tile C type pelvic fractures

Objective To explore the therapeutic effect of percutaneous iliosacral screws combined with external fixation in the treatment of Tile C type pelvic fractures. Methods The therapeutic effects of patients with Tile C type pelvic fracture underwent percutaneous iliosacral screws combined with external fixation (study group, 30 cases) and single external fixation (control group, 33 cases) were retrospectively analyzed, the postoperative Matta scores and Majeed scores of the two groups were compared, and the adverse reactions of patients in the two groups were also observed. Results All the 30 patients in study group achieved primary healing; and 27 of 33 patients in control group achieved primary healing, the other 6 patients achieved per secundam intentionem. The excellent rates of postoperative Matta scores and Majeed scores of study group (90.0%, 90.0%) were statistically higher than those of control group (75.8%, 72.7%) (χ2=7.64, 6.08, P < 0.05). No one adverse reaction was observed in the study group. 8 patients occurred displacement of restoration, and these 8 patients were healed after altered to percutaneous iliosacral screws combined with external fixation. Conclusion Percutaneous iliosacral screws combined with external fixation is characterized by small injury, safe and effective in the treatment of Tile C type pelvic fractures, and it has no obvious adverse reactions, thus it is worthy of extensive applied in clinic.
