健康人群肝内强回声病变超声显像分析

目的探讨健康人群肝内强回声病变的声像图特点、鉴别诊断及临床意义。方法采集2009～2011年参加健康体检的9717名行政事业单位职工常规腹部超声结果,对1823例肝内强回声病变超声显像进行回顾性分析。结果 1823例肝内强回声灶多位于肝右叶,以肝内钙化性病变最常见。其中,肝内钙化灶1050例,孤立,散在分布,无胆管扩张,后方可伴声影;肝内管壁局限性钙化588例,呈"等号样"强回声,无胆管扩张,后方声影模糊或不明显,两者形态、位置稳定;肝内胆管结石131例,好发于左叶,多发常见,与门脉伴行,远端胆管扩张,大多伴声影,形态、位置相对稳定;肝内胆管积气51例,胆管扩张常见,后方伴慧星尾征,形态位置易改变。结论超声对肝内强回声灶的诊断与鉴别具有重要的临床意义,既可提高健康体检超声检查的质量,又能减少患者不必要的经济负担及思想压力。

Ultrasonoscopy analysis on strong echo lesions inside liver of healthy people

Objective To discuss the characteristic of ultrasonogram,differential diagnosis and clinical significance of the strong echo lesions inside liver of healthy people.Methods The conventional belly ultrasound consequence on physical examination of the 9 717 staffs in administrative institution were collected from 2009 to 2011,and 1 823 cases of ultrasonogram of strong echo lesions inside liver were retrospective analysised.Results Most of the 1 823 liver echo focals situated in the right lobe of liver,the intrahepatic calcification was the most common pathological changes.Among them,1 050 cases with calcification insided the liver,and the calcifieds within the liver were isolated and scattered,no bile duct expan-sion and the sound with shadows in the rear;588 cases with limitations calcification inside liver wall,and present strong equals sample echo,there was no bile duct expansion,and the acoustic shadow in the rear was ambiguous or inconspicuous,both of the shapes and positions were stable;131 cases with hepatolithiasis in the left lobe of liver,which was frequent and common,and standed with portal vein,remote bile duct expansion with sound shadow at the rear mostly,the shapes and positions were stable relatively;51 cases with pneumatosis in intrahepatic bile ducts,and the biliary ducts were expanded frequently with comet tail at the rear,the shapes and positions were easy to change.Conclusion There is clinical significance to the diagnosis and identify on the liver echo focal by ultrasound,not only it can enhances the quality of ultrasonic examination in healthy examination,but reduces the unwanted financial burden and thought pressure on the patient as well.
