脊柱内固定术后感染的研究

目的 探讨脊柱内固定术后感染的危险因素及围手术期处理对感染的预防作用。方法 回顾性分析186例脊柱内固定手术病人手术部位、手术时间、失血量及围手术期抗生素的使用等因素对术后感染的影响。结果 186例病人术后感染发生率为4．30％(8／186)，颈椎、胸框、腰椎部位手术术后感染发生率依次为1．51％、14．29％、3．26％；手术时间60min以内、60min—和120min—术后感染发生率依次为1．22％、3．03％和13．16％；术中失血量50m1—、400m1—和800m1—术后感染发生率依次为2．15％、3．33％和12．12％；术中使用抗生素，术中和术后使用抗生素，术前、术中和术后均使用抗生素术后感染发生率分别为1．20％、4．48％、11．11％。结论 脊柱内固定术后感染的发生与手术部位、手术时间、失血量有关，以胸框部位手术术后感染发生率较高，手术时间越长、失血量越多术后感染越易发生，围手术期抗生素的使用可有效减少感染的发生。

Study on post-operative infection of spinal internal fixation surgery

Objective To study the risk factors and preventive methods of post-operative infection of spinal internal fixation surgery.Methods The operation site,operation time,blood lose and antibiotics administration in 186 patients underwent internal fixation surgery were analyzed retrospectively.Results The post-operative infection rate of the patients was 4.30%(8/186),and the infection rates of the patients operated in cervical,thoracic and lumbar were 1.51%,14.29% and 3.26% respectively.When the operation time were less than 60min,more than 60min and 120min,the infection rates were 1.22%,3.03% and 13.16% respectively.Administration of antibiotics in operation,both in operation and post-operation,and in pre-operation,operation and post-operation,the infection rates were 1.20%,4.48% and 11.11%,respectively.Conclusion The occurrence of post-operative infection of spinal internal fixation surgery was related to operation site,operation time and blood lose,and antibiotics may be an effective method for prevention of infection.
