| 摘 要： | 目的探讨膀胱小细胞癌的临床病理特征及预后因素,提高对该疾病的认识和诊治水平。方法总结1989年1月~2005年7月国内已报道的49例膀胱小细胞癌临床资料,结合文献分析该肿瘤的组织来源、病理和免疫组织化学特征、诊治并分析各种临床病理特征与生存率相关性。结果49例膀胱小细胞癌中,男女比例4.44∶1,60岁以上男性多见。临床症状以肉眼血尿为主,肿瘤多位于膀胱侧壁,直径通常<3.0cm。病理检查见肿瘤多为混合细胞型。国内治疗以手术为主,化疗放疗其次。临床病理特征如年龄、性别、症状、肿瘤发生位置、肿瘤大小及细胞成分、免疫组化特征与其生存关系没有显著性差异。患者生存时间与膀胱切除术治疗之间也未见统计学意义(P>0.05)。结论膀胱小细胞癌是一种比较少见的高度恶性肿瘤,有独特的病理形态。血尿为主要临床表现。早期即可发生转移,预后差。确诊依靠病理检查及免疫组化,手术切除联合放疗和化疗是主要的有效治疗手段。早期发现和治疗是提高生存率和延长生存时间的关键。
