改良双环法加悬吊固定术治疗乳房下垂和乳房肥大

目的探讨改良双环法加悬吊固定术在乳房下垂、乳房肥大整形中的应用。方法设计时站立位与仰卧位相结合,注意双侧对称性,以乳房下皱襞与乳房子午线交点、正中线、乳腺外侧缘这几个位置恒定的点或线作为标志画出外环线,以乳头为中心画直径3.5~4.0cm圆圈为内环线,切除双环间表皮;仅剥离乳腺上象限,对乳房下垂进行悬吊、固定、塑形。结果采用本方法矫治轻、中度乳房下垂、乳房肥大患者共13例26侧,术后乳房外形美观持久,乳头、乳晕血运正常,感觉良好,效果满意。结论在轻、中度乳房下垂及乳房肥大的整形术中,利用改良双环法加悬吊固定术具有设计合理、操作简单、创伤小、效果好等优点,尤适于门诊开展。

Application of modified "round block" technique mastopexy in correction of mastoptosis and hypermastia

Objective To review and discuss the application of modified "round block" technique mastopexy in the correction of mastoptosis and hypermastia. Methods Both standing and supine positions and bilateral symmetry were considered when design of operation. The external ring was drawn according to the cross of mammary inferior plica and meridian, the median line, the lateral edge of the breast, which were comparatively steady. Taking the nipple as the central point, we drawn the internal ring, whose diameter was between 3.5 to 4.0 cm. The epidermal tissue was incised between the outside and the inside ring. The upper quadrant of the breast was just peeled off, then suspensed, fixed and moulded. Results A total of 26 breasts in 13 patients with mild to moderate mastoptosis and hypermastia were treated with the present technique. Satisfactory results were achieved, such as pretty breast shape, enough blood supply and proper sense of nipple aerola. Conclusion In the correction of mild, moderate mastoptosis and hypermastia, the modified "round block" technique mastopexy has many advantages, such as suitable design, easy operation, slight impairment, satisfying effects, and especially fit for the outpatient clinic.
