123例乳腺导管内癌临床特点和远期疗效

目的：探讨乳腺导管内癌的临床病理特点和治疗方式。方法：回顾性分析123例乳腺导管内癌病人的发病年龄、临床分期、原发癌表现和治疗方式与预后的关系。结果：123例病人经治疗后，7例非乳腺癌原因死亡，2例因乳腺癌死亡，其余病人均健在；不同术式组预后无统计学意义；加辅助治疗与单纯手术组预后无统计学意义。结论：导管内癌的治疗应根据发现方式、病变大小、组织学特点等因素综合设计个体化治疗；除非临床高度怀疑有区域淋巴结转移，外科治疗应以较保守术式为宜；核分级为Ⅲ级者预后不佳，应采取根治性手术和积极的综合治疗。

Clinical characters and long-term treatment effect on 123 cases of ductal carcinoma in situ of breast

Objective: To explore the clinical characteristics and treatment of DCIS of breast. Methods: To retrospectively analyse the relationship between age, clinical stage, primary tumor's representation, treatment and prognosis on 123 cases of DCIS of breast.Results: In all of 123 cases, 2 cases were died of breast cancer, 7 cases were died of non-relationship with breast disease, and the others were still living and in good health. There was no significant difference among different types of operation. Prognosis of operation group and adjuvant therapy group had no significant difference. Conclusion: Treatment of DCIS should be based on primary tumor's representations, clinical stage and histologic characters. Conserving treatment is suitable for DCIS, except that the patient is with suspicious positive axillary lymph node. Patients with higher nuclear grade have poorer prognosis, and should be performed mastectomy and combined treatment actively.
