肝素结合性表皮生长因子在翼状胬肉中的表达及意义

摘    要:目的 研究肝素结合性表皮生长因子(heparin-bindingepidermalgrowthfactor,HB-EGF)在翼状胬肉组织中的表达与分布,探讨其在翼状胬肉发生过程中的作用。方法 收集2013年8月至2014年8月在南昌大学眼科医院行翼状胬肉切除的40例标本,采用免疫组织化学染色和Westernblot分别检测22例初发与18例复发翼状胬肉标本中HB-EGF的表达情况,并与正常对照组结膜标本进行比较。结果 HB-EGF在初发型翼状胬肉组织的头部相对表达值为2.77、体部相对表达值为1.76,正常结膜相对表达值为1.37。复发型胬肉组织中HB-EGF在其头部相对表达值为1.95、体部相对表达值为1.57,正常结膜相对表达值为0.96。HB-EGF在初发型和复发型翼状胬肉组织中均主要定位于上皮细胞和血管内皮细胞的胞浆中,以上皮细胞表达最为显著。相同标本数复发型翼状胬肉头部组织中HB-EGF的相对表达值为4.30,为正常对照结膜(相对表达值为1.09)的3.9倍、为初发型(相对表达值为1.32)翼状胬肉头部组织3.1倍,且初发型翼状胬肉头部组织表达量为正常对照组的1.3倍。结论 高水平的HB-EGF可能是翼状胬肉发生与复发的重要因素,提示可以把HB-EGF作为干预靶点来控制翼状胬肉的发生与复发。

Abstract:Objective To investigate the expression and distribution of heparin-binding epidermal growth factor ( HB-EGF) in pathogenesis of human pterygium, and study the role of its played. Methods Between August 2013 and August 2014 ,40 cases of pterygium exasion specimens were collected and produced in Affiliated Eye Hospital of Nanchang University. The expression differences of HB-EGF between pterygium and normal conjunctiva tissue from the same pterygium patient were compared with immunohistochemistry and Western blot. Also we used the same technique to examine the expressions and distribution of HG-EGF in pterygium tissue of 22 cases with first operation and 18 recurrent cases as well as the expression in 15 specimens of normal conjunctiva from patients operated for other disorders. Results In primary pterygium group , the relative intensities of HB-EGF in pterygium-head was 2. 77. pterygium-body was l. 76 and normal conjunctiva tissue was l. 37. In recurrent pterygium group ,the relative intensities of HB-EGF in pterygiumhead was l. 95 , pterygium-body was l. 57 and normal conjunctiva tissue was 0. 96. And HB-EGF largely distributed in the endochylema of epithelial cells and vascular endothelial cells of pterygium , especially in epithelial cells. The relative intensities of total HB-EGF in recurrent group was 4. 30 which was 3. 9 times over the normal conjunctive group ( the relative intensities was l. 09 ) .3. I times as strong as the primary group ( the relative intensities was l. 32) . and the primary group was l. 3 times over the normal conjunctive group. Conclusion Over-expression of HB-EGF is an important cause of development and recurrence of pterygium, which indicate that HB-EGF can be set as the therapeutic target for controlling the development and recurrence of pterygium.
