吻合器痔上黏膜环切钉合术治疗环形混合痔的疗效

目的 探讨吻合器痔上黏膜环切钉合术(PPH)治疗环形混合痔的临床效果.方法 将68例环形混合痔患者按随机数字表法分为2组,每组34例.对照组采用常规外切内扎术(MMH)进行处理.PPH组采用环形痔吻合器行PPH治疗.对2组的临床效果(手术时间、出血量、伤口愈合时间、住院时间及住院费用)及术后并发症(便后出血、术后疼痛、肛缘水肿、肛门分泌物及瘙痒、尿潴留)发生情况进行比较.结果 与对照组比较,PPH组手术、住院时间更短,伤口愈合更快、术后并发症更少,但住院费用相对更高(均P＜0.05).结论 PPH术在治疗环形混合痔方面具有手术时间短、伤口愈合快、住院时间短和术后并发症少等特点,可作为环形混合痔治疗的较好选择.

Effect of Procedure for Prolapse and Hemmorrhoids on Annular Mixed Hemorrhoids

Objective To investigate the efficacy of the procedure for prolapse and hemmorrhoids （PPH） in the treatment of annular mixed hemorrhoids. Methods Six-eight patients with annular mixed hemorrhoids were randomly assigned to receive either Milligan -Morgan hemorrhoidectomy （control group, n=34） or PPH （PPH group, n=34）. Clinical effects （operative time, blood loss, wound healing time, length of stay and hospital costs） and postoperative complications （hematochezia, postoperative pain, perianal edema, anal secretions, anal pruritus and urinary retention）were compared between the wound healing, reduced postoperative complications but increased hospital costs （P〈0.05）. Conclusion PPH is a better choice than Milligan-Morgan hemorrhoidectomy in the treatment of annular mixed hemorrhoids with shorter operation time and hospital stay, more rapid wound healing and fewer complications.
