关于脐尿管癌临床病理学特点及手术疗效的研究

目的：探讨脐尿管癌的临床病理学特点及手术方式，提高其诊疗水平。方法：回顾性分析总结10例脐尿管癌患者的临床和病理资料。10例中男8例，女2例，平均年龄53岁。症状包括间歇性无痛肉眼血尿8例，血尿伴尿中有黏液1例，血尿伴下腹疼痛1例。术前检查包括泌尿系B超、膀胱镜检、尿瘤细胞、CT和静脉尿路造影。10例中行扩大性膀胱部分切除术9例，行膀胱部分切除术1例。肿瘤直径2—7cm，平均3．7cm。结果：本组10例中病理报告为黏液腺癌8例；未特殊分类腺癌l例；小细胞型内分泌癌1例。Sheldon分期ⅢA期9例，ⅢC期1例。1例术后3个月死于多发性骨转移，1例术后13个月因膀胱前壁局部复发行二次手术，术后随访6个月，无肿瘤复发，1例分别于术后15个月和2年出现膀胱颈部及尿道复发行经尿道肿瘤电切术，其余7例随访2～36个月，均无肿瘤复发。结论：对脐尿管癌早期行扩大性膀胱部分切除术可达到长期无瘤生存效果，熟悉本病的临床病理学特点对于提高诊断水平非常重要。

Urachal Carcinoma:clinicopathologic features and surgical effects
