摘    要:目的:研究16排螺旋CT后处理技术在气管、支气管树成像中的应用价值和限度。材料和方法:搜集接受MSCT肺部扫描的正常组50例,病变组50例,均行16排螺旋CT薄层容积扫描,进行多层面/曲面重建(MPR/CPR)、最小密度投影(MinIP)、表面阴影遮盖法(SSD)和仿真支气管内窥镜成像(VB),在不使用造影剂的情况下,行气管、支气管树立体成像。比较4种后处理方法对气道显示的差异和发现病变的敏感性等。利用SPSS10.0统计分析软件系统进行分析。结果:(1)4种后处理方法对气道显示均可达到亚段(4级)或以下支气管,对各种解剖及相互关系的显示各有优点。(2)MinIP、SSD和VB对病变的显示直观,整体感好。(3)SSD和VB对2、3、4级气道的评估存在误差,且差异有显著性(P〈0.05)。(4)MPR/CPR、MinIP、SSD和VB发现气道病变的敏感性分别为90.0%、80.0%、60.0%和55.0%。结论:16排MSCT四种后处理重建技术结合横断面图像是评价和诊断气管、支气管气道病变的理想的影像学方法。

Clinical Application of Postprocessing Techniques of 16-detecter Spiral CT Images in the Tracheobronchography

Abstract:Objective To evaluate the value of multiplanar reconstruction of 16-detecter Spiral CT Images in the Tracheobronchography. Methods Four kinds ofpostprocessing of 16 detector spiral CT including MPR, MinIP, SSD and VB were performed in fifty normal volunteers and fifty patients. The differences of airway appearances and measurements, diagnostic sensitivity for bronchotracheal lesions of the 4 methods were compared. Results (1) Four kinds of postprocessing were able to demonstrate second segmental even smaller bronchial divisions. (2) MinIP, SSD and VB can directly reveal the whole of the bronchotracheal lesions. (3) The measure of the normal bronchi showed there were significant diffrence between SSD and VB with MPR (p〈0.05). in the assess of 2, 3, 4grade airway. (4) The sensitivities ofMPR, MinIP, SSD and VB for detecting bronchotracheal lesions were 90.0%, 80.0%, 60.0% and 55.0%. Conclusion It is helpful for four kinds ofpostprocessing of 16 detector spiral CT to diagnose bronchotracheal lesions.
