中药内服与高位保留灌肠治疗急性胰腺炎40例疗效观察

目的观察中药内服与高位保留灌肠治疗急性胰腺炎的临床疗效。方法将80例急性胰腺炎患者随机分为对照组与治疗组各40例,对照组禁食,胃肠减压,纠正水、电解质及酸碱平衡失调以及静脉营养支持,并给予生长抑素、制酸剂、抗生素抗感染治疗。治疗组在对照组治疗的基础上,同时内服清胆宁胰汤(2次/d)及三黄栀子汤高位保留灌肠(1次/d),疗程7d。观察两组治疗后发热、腹痛腹胀、血尿淀粉酶的变化、恢复排气排便时间及B超改变比较。结果两组治疗后发热、腹痛、腹胀、排气排便的恢复,B超及总有效率均有所改变,而治疗组改变更加明显(P0.05),两组比较有显著性差异。结论采用内服清胆宁胰汤和三黄栀子汤高位保留灌肠治疗急性胰腺炎,有减少胰腺分泌,促进功能恢复的作用,疗效确切,具有良好的临床意义,值得推广。

Clinical Observation on Oral Administration of Traditional Chinese Medicine and High Retention Enema in Treatment of Acute Pancreatitis

Objective To observe the clinical effect of oral administration of Chinese medicine and high retention enema in treatment of acute pancreatitis.Methods 80 acute pancreatitis patients were randomly divided into control group and treatment group,40 cases in each group.Control group was treated with fasting,gastrointestinal decompression,correction of water,electrolyte and acid-base imbalance,as well as intravenous nutritional support,and given somatostatin,antacids,antibiotics,anti infection treatment.Treatment group was treated with orally-taking Qingdan Ningyi decoction(2/d) and Sanhuang Zhizi decoction high retention enema (1/d) for 5-7d on the basis of control group.The fever,abdominal pain,abdominal distension,hematuria amylase changes were compared after treatment between the two groups,the recovery time of exhaust defecation as well as B ultrasound changes were also compared.Results The fever,abdominal pain,abdominal distension,exhaust defecation recovery,B ultrasound and total effective ratio were changed after treatment in both groups,and treatment group was more obvious;there was significantly difference between the two groups (P ＜0.05).Conclusion Qingdan Ningyi decoction combined Sanhuang Zhizi decoction high retention enema is effective in treatment of acute pancreatitis,it can reduce pancreatic secretion,curative effect,promote the functional recovery of the exact with good clinical significance,which is worthy of promotion.
