盆腔脏器联合切除治疗泌尿系肿瘤

目的探讨盆腔脏器联合切除术在治疗泌尿系肿瘤中的作用。方法对行联合脏器切除的9例泌尿系肿瘤患者（包括前列腺肉瘤术后复发1例、前列腺肉瘤术后胃肠道间质瘤转移1例、前列腺肉瘤1例、前列腺癌术后复发1例、宫颈癌术后复发1例、膀胱肉瘤1例、直肠癌术后复发3例）进行回顾性分析。结果 8例接受全盆腔脏器切除术,1例为根治性膀胱前列腺切除。无围手术期死亡,术后并发症发生率100%。随访至今,3例健在,平均生存时间26.8（5～64）个月。结论盆腔脏器联合切除可有效地治疗周围脏器受侵犯的泌尿系肿瘤,改善患者预后,提高生活质量,但术后并发症较多。患者预后主要与原发肿瘤病理类型有关。

Pelvic exenteration for tumors of the urinary syste

Objective To discuss the treatment meaning of the pelvic exenteration for the genito-urinary tumors. Methods Clincal datas of 9 patients who underwent pelvic exenteration were analyzed retrospectively （2 prostate sarcoma, 1 bladder leiomyosarcoma, 1 relapse prostate cancer, 1 gastrointestinal stromal tumor, 1 cervical cancer, 3 relapse rectal cancer）. Results 8 patients underwent total pelvic exenteration, 1 eystoprostatectomy. No perioperatipn death. All pa- tients had postoperation complications. The mean survival time was 26.8 （5-64） r~onths, 3 still a- live. Conclusions Pelvic exenteration can be used to treat the genito-urinary tum~）rs which invade the surrounding organs, prolonging survive time, improving quality of life, but there are more posts- urgieal complications. The prognosis is mainly related to the pathology of the primary tumor.
