血清冷球蛋白定量检测在丙肝肝硬化诊断中的应用研究

目的:探讨血清冷球蛋白(CG)在丙肝肝硬化诊断中的价值。方法:收集丙型肝炎病毒(HCV)感染患者45例,丙肝肝硬化23例,正常对照50例,分别检测其血清CG含量。将患者按不同年龄、性别及携带HCV病毒不同持续时间的CG含量进行统计分析;血清CG定量与透明质酸(HA)、层粘蛋白(LN)、型前胶原(PC)及型前胶原(PC)进行相关性分析。结果:CG>293.2mg/L组中,HCV感染阳性率为77.8%,丙肝肝硬化阳性率为78.3%;丙肝肝硬化发生率为男性27.8%,女性为28.1%。在丙肝肝硬化组中,>60岁组CG定量为563.2mg/L;携带HCV病毒>15年,CG定量为575.2mg/L。在丙肝肝硬化组A、B、C三级中CG定量有显著性差异。结论:CG检测有助于慢性HCV感染及丙肝肝硬化患者的早期诊断。

The application of serum CG guantification in the diagrosis of hepatic cirrhosis derived from C hepatits

Objective：To investigate the diagnostic values of serum cryoglobulin（CG）in patients with hepatitis C virus（HCV）infection complicated with hepatic cirrhosis.Methods：A turbidity method was used to detected the serum CG concentrations from 45 patients with HCV infection,23 patients with HCV infection complicated with hepatic cirrhosis.50 healthy donors were used as normal controls.Patients were divided into different groups according to age,gender and duration of HCV infection.Statistical significance of serum CG level between each group was determined using Mann-Whitney U test.A correlation analysis was performed between CG level and other clinical index,including HA,LN、PCⅢ as well as PCⅣ.Results：CG293.2mg/L,positive rate of HCV infection was 77.8%,positive rate of HCV infection complicated with hepatic cirrhosis was 78.3%.Incidence rate of HCV infection complicated with hepatic cirrhosis was 27.8%in male and 28.1% in female.CG was measured 563.2 mg/L in exceed 60 age of patients with HCV infection complicated with hepatic cirrhosis;patients with HCV infection exceed 15years,CG was measured 575.2 mg/L.The detection showed significant difference among the A,B,C there degrees in the group with hepatic cirrhosis.Conclusion：Measurement of CG may help to diagnose in patients with chronic HCV infection.
