近视眼RNFL光学相干断层扫描测量及相关因素分析

摘    要:目的 探讨近视眼视网膜神经纤维层(RNFL)厚度的变化特点及相关影响因素.方法 将行准分子激光原位角膜磨镶术(Lasik)术前检查的患者109例213眼按近视度数的高低分为3组低度近视组51例,中度近视组91例,高度近视组71例.分别测量3组患者各个象限的RNFL厚度、视盘面积、盘沿面积、杯盘面积比、眼压、瞳孔直径,所有数据采用SPSS13.0统计软件进行分析.结果 低度近视组患眼上象限RNFL厚度(137.216 4-19.014)μm与中度近视组患眼上象限RNFL厚度(136.549 4-18.507)μm比较差异无统计学意义(P>0.05),其余各近视组患眼的各象限RNFL厚度差异均有显著统计学意义(均P<0.01).上、下、鼻象限RNFL厚度与近视度数呈正相关(r=-0.259、-0.316、-0.183.均P<0.01).颞侧RNFL厚度与近视度数呈负相关(r=0.318,P<0.01);RNFL平均厚度与眼压、瞳孔直径无相关性(r=-0.088、0.050,均P>0.05),与视盘面积、盘沿面积呈正相关(r=0.165、0.262.P<0.05或0.01),与杯盘面积比呈负相关(r=-0.140,P<0.05).结论 上、下、鼻象限RNFL厚度及平均厚度均随近视度数的增高而变薄,颞侧RNFL厚度随近视度数增加而逐渐增厚.RNFL平均厚度随着视盘面积、盘沿面积的增大而增厚,随着杯盘面积比增大而变薄,与眼压、瞳孑L直径无显著相关性.
