MRI动态增强在判断子宫内膜癌肌层浸润中的应用价值

目的:探讨MRI动态增强在判断子宫内膜癌肌层浸润中的应用价值。材料与方法:回顾性分析33例经手术病理证实的子宫内膜癌的MR动态增强表现,观察肿瘤的强化特点、内膜下强化带的显示及肌层内表面情况,将MR动态增强判断肌层浸润的结果与病理比较。结果:动态增强延迟期31例子宫内膜癌呈轻至中等强化,与肌层信号比较,呈相对低信号,2例子宫内膜癌明显强化;动态增强早期显示子宫内膜下强化带22例,不出现内膜下强化带者,肌层内表面不规则10例,光滑1例。以手术病理为准,MRI动态增强扫描判断肌层浸润的诊断符合率为90·9%。结论:MRI动态增强对判断子宫内膜癌肌层浸润有较高的价值,应列为术前MRI检查常规序列。

The Application Value of Dynamic Contrast- enhanced MRI im Judging the Myometrial Invasion of Endometrial Carcinoma

Purpose:To study the application value of dynamic contrast-enhanced MRI in judging the myometrial invasion of endometria carcinoma.Materials and Methods:The dynamic contrast-enhanced MRI features were retrospectively analyzed in 33 cases of endometrial carcinoma proved by operation and pathology.The enhancement feature of the tumors,and the appearance of subendometrial enhancement zone,and the inside surface of myometrium were observed.The dynamic contrast-enhanced MRI diagnosis of myometrial invasion were correlated with pathologicv findings.Results:In the delayed phase of dynamic contrast-enhanced sequence,31 cases of endometrial carcinoma showed mild or moderate enhancement,and 2 cases tended to enhance markedly.On the early phase of dynamic contrast-enhanced images subendometrial enhancement zone was seen in 22 cases.The inside surface of myometrium appeared as Irregular in 10 cases and sharp in 1.The accuracy of dynamic contrast-enhanced MRI were 90.9% for assessing myometria invasion.Conclusion:Dynamic contrast-enhanced MRI have considerable value in judging the myometrial invasion of endometrial carcinoma,and should be performe as a routinel sequence of MRI examination for endometrial carcinoma before operation.
