七氟醚吸入联合腋路臂丛神经阻滞在小儿手外科手术中的应用

目的观察七氟醚吸入复合腋路臂丛神经阻滞在小儿手外科手术中的应用,并与氯胺酮静脉麻醉进行比较。方法选择行手外伤,上肢手术的患儿40例,随机分为七氟醚吸入复合腋路臂丛神经阻滞组（S组）和氯胺酮（K组）,每组20例,记录氧饱和度（SpO2）、心率（HR）、呼吸频率（RR）、麻醉诱导时间、疼痛反射消失时间、清醒时间及恶心呕吐等不良反应的发生率。结果S组患者术中SpO2变化不明显,而K组术中SpO2变化明显,S组患者术中HR、RR变化均不显著,K组HR、RR均明显升高,S组麻醉诱导时间、疼痛反射消失时间、清醒时间均较K组缩短,S组术后恶心呕吐发生率明显低于K组。结论七氟醚复合腋路臂丛神经阻滞用于小儿手外科手术的方法安全有效,简单易行。

The application of sevoflurane inhalation combined with armit way brachiplex nerve block in children hand surgery

Objective To observe the application of sevoflurane inhalation combined with armpit way brachiplex nerve block in children hand surgery and comparison with ktamine intravenous anesthesia. Methods 40 children who suffered hand injury and would accept operation were involved in this study. They were separated into sevoflurane inhalation combined with armpit way brachiplex nerve block group（S group） and ketamine group （ K group） （ n = 20）,randomly. During operation,Pulse Oxygen Saturation （ SpO2 ）,Heart Rate （HR）,Respiration Rate （RR）,anesthesia induction time ,nocicepeive reflex disappear time,awake time and the incidence rate of adverse effect after operation,such as nausea ,vomiting and so on,were recorded. Results In S group,SpO2,HR and RR had no significant change （ P 0. 05） ,However ,in K group,SpO2 changed and HR,RR increased significantly （P 0. 05）. Anesthesia induction time,nociceptive reflex disap-pear time and awake time in S group were shorter than those in K group （P 0. 05）. The incidence rate of ad-verse effect after operation in S group was lower than that in K group significantly （P 0. 05）. Conclusion Sevoflurane inhalation combined with armpit way brachiplex nerve block is a safe ,effective ,simple and con-venient method in children hand surgery.
