大鼠急性脑梗死溶栓治疗时间窗的研究

摘    要:目的 研究尿激酶溶栓治疗大鼠急性脑梗死的时间窗。方法 用自体血栓法制作大鼠大脑中动脉闭塞 (MCAO)模型 ,在栓塞后 30、6 0、90、12 0、180min(A、B、C、D、E组 )经静脉注射尿激酶 (5万U/kg)溶栓 ,用神经功能缺损评分、TTC染色测梗死体积、核磁共振 (MRI)及病理学观察 ,比较不同时间点溶栓的疗效及安全性。结果 MCAO后 90min内溶栓 (A、B、C组 )能显著改善神经功能 ,缩小梗死体积 (P <0 .0 5 ) ,12 0min以后溶栓组 (D、E组 )与对照组无显著性差异 (P >0 .0 5 ) ,且并发脑出血率高 (31.3% )。结论 本研究提示大鼠脑梗死溶栓治疗最佳时间窗为栓塞后 90min内 ,溶栓治疗时间越早 ,疗效及安全性越高。

Abstract:Objective To investigate the time window of urokinase(UK) thrombolysis.Methods The middle cerebral artery occlusion(MCAO) model was induced in rats by autologous clot, the UK (50000 U/kg) was injected intravenously at 30th (group A)\,60th(B)\,90th(C)\,120th(D)\,180th(E) minute after MCAO respectively. The efficacy and safety of UK were evaluated by neurologic deficit score, infarct volume and magnetic resonance imaging (MRI).Results Both neurologic deficit score and infarct volume were significantly improved by injected UK within 90 minutes after MCAO( P < 0.05 ). The infarct size didn't reduce, however, the rate of cerebral hemorrhage increased ( 31.3% ) when treated by UK at 120th or 180th minute.Conclusion Our study indicated that optimal therapeutic window of UK thrombolysis in acute infarct of rats is less than 90 minites after thrombosis.
