侧裂深部脑膜瘤的手术治疗（附5例报告）

报告了5例侧裂深部脑膜瘤的手术治疗结果。4例疗效满意,1例死于术后脑梗塞和脑水肿。指出,术中保护好大脑皮层,不损伤和过多地刺激脑中动脉及其分支是该手术成功的关键。文章还就侧裂深部脑膜瘤的解剖学特点、临床表现以及放射学检查进行了讨论。

FIVE CASES OF DEEP SYLVIAN MENINGIOMA

Five cases of deep sylvian meningioma we treated since 1980 are presented with some considerations of their anatomical, clinical and radiological features. These meningiomas were located in the extra-axial space of the sylvian fissure without any attachment to the dura mater or the choroid plexus of the ventriocles. All of the cases showed relatively definite signs, for the tumor depressed the island lobe of brain and affected the adjacent cerebral structures. Cerebral angiography often showed intrasylvian mass with tumor stain, but the external carotid artery was typically not involved. Now it is easy to define its location by GAG and CT. Our cases were finally diagnosed surgically and pathologically. Protection of both the cerebral cortex and the middle cerebral artery and its branches is a key to successful operation. The operative results were excellent in 4 cases, but one patient died of cerebral embolism and edema after operation.
