MED与开放手术治疗腰椎间盘突出症的疗效比较

摘    要:目的比较显微内窥镜椎间盘切除系统(microendoscopy discectomy,MED)与开放腰椎间盘摘除术的临床疗效.方法采用回顾性分析方法,对70例单一水平腰椎间盘突出症患者行MED手术和同样例数同类患者接受传统手术进行比较研究.根据macnab标准评价疗效,并比较手术时间、术中出血量、术后镇痛药物应用、术后下床活动时间及住院日.结果MED组优良率为92.9%,开放手术组优良率95.1%,而在术中出血量、术后痛痛药物的应用、住院时间及术后下床活动时间上,统计学上有显著差异.结论两种方法疗效相近,但椎间盘镜下椎间盘切除术能明显减少术中的出血量、住院时间、手术后下床活动时间及术后镇痛药物的应用,是一种治疗单一节段腰椎间盘突出症的较理想的手术方法.

Abstract:Objective: To compare the clinical results of microendoscopy discectomy with opening traditional opened discectomy. Methods: With the methods of retrospective study with randomized block design to analysis the operation time, the blood loss in operation, weather to use anodyne. The time in hospital and the time out of bed after operation. In the 70 patients suffered from only one lumber disc heniation operated by MED compared with the same disease, samples operated by traditional opened operation. According to macnab method. Results: The excellent and good rate was 92.9% in MED group and 95.1% in control group but there was significant difference in statistics about the blood loss in operation, the using anodyne, the time in hospital and the time out of bed after operation between the two groups. Conclusions: The clinical result is nearly same with the two methods but MED can reduce the blood loss in operation, using anodyne, the time in hospital and the time out of bed after operation, that is safe and effective in the treatment of only one lumber disc heniation.
