摘 要: | 目的探讨e-Flow成像技术在甲状腺良恶性肿瘤鉴别诊断中的应用价值。方法选取2011年6月至2012年12月河北北方学院附属第一医院收治的甲状腺肿瘤患者60例,其中甲状腺腺瘤31例(甲状腺腺瘤组),甲状腺癌29例(甲状腺癌组),术前应用e-Flow成像技术检测肿瘤内部血流情况,术后应用免疫组织化学技术统计肿瘤微血管密度(MVD),分析血管组织形态、MVD与肿瘤类型的关系。结果①甲状腺癌组血流分级以Ⅱ、Ⅲ级为主,甲状腺腺瘤组血流分级以0、Ⅰ级为主,差异有统计学意义(P<0.05);②两组MVD值比较,甲状腺癌组MVD值高于甲状腺腺瘤组,差异有统计学意义(P<0.05);③甲状腺肿瘤的血流分级与CD34、CD105标记的甲状腺肿瘤血管形态比较,低血流分级(0、Ⅰ级)以点状、线状血管为主,而高血流分级(Ⅱ、Ⅲ级)以带状、网状血管为主,差异有统计学意义(P<0.05)。结论甲状腺良恶性肿瘤的血管病理学特征和血流信号分级存在明显差别,e-Flow成像技术在临床鉴别诊断中具有重要价值。

Abstract: | Objective To investigate the clinical value of e-Flow imaging techniques in the differential diagnosis of thyroid benign and malignant tumors. Methods A total of 60 cases with thyroid tumor were randomly selected from the First Attached Hospital of Hebei Northern University during Jun. 2011 and Dec. 2012 and divided into two groups,e-Flow imaging was used to detect blood flow inside the tumor before the operation,and immunohistochemical techniques were applied to mark tumor microvessels and statistically analyze its density ( MVD), and the relationship between MVD, vascular morphology and tumor type were analyzed. Results (1)Flow grade Ⅱ and Ⅲ were predominant in the thyroid cancer group, flow grade O, I in thyroid adenoma group, and there was a significant difference between the two groups (P 〈 0.05 ) ;(2)as for MVD value comparison, thyroid cancer group had a higher MVD value than the thyroid adenomas group, with statistically significant difference( P 〈 0.05 ) ; (3) thyroid tumors' flow grade correlation with CD34, CD105 marked thyroid tumors vascular morphology, low-flow grade (O, I level) was mainly correlated with point and line blood vessels ,high-flow grade ( Ⅱ , Ⅲ) was mainly correlated with ribbon and mesh blood vessels, which showed a statistically significant difference(P 〈 0.05 ). Conclusion Vascular pathology characteristics and flow signals classification are significantly different between thyroid benign and malignant tumors and e-Flow imaging technology has important value in the clinical differential diagnosis. |
