摘    要:目的探讨改良短时受精在体外受精一胚胎移植(IVF—ET)周期治疗中对胚胎质量的影响及其I临床应用价值。方法回顾性分析2009年9月至2012年8月在唐山市妇幼保健院生殖中心接受体外受精助孕治疗的患者886例,其中短时受精不脱颗粒细胞组(A组)235例,短时受精并全部即刻脱颗粒细胞组(B组)315例,短时受精并部分(3~5枚)脱颗粒细胞组(c组)336例。分别比较3组的受精率、多精受精率、卵裂率、优质胚胎率、妊娠率、胚胎种植率和流产率。结果3组的受精率、卵裂率、胚胎种植率和流产率差异无统计学意义(P〉0.05);B组的多精受精率高于A、C两组(P〈0.05);C组的优质胚胎率和临床妊娠率高于A、B两组(P〈0.05)。结论短时受精并部分即刻脱颗粒细胞不影响受精率、卵裂率和胚胎种植率.但可提高优质胚胎率及临床妊娠率,并且能够降低多精受精率。

Abstract:Objective To evaluate the methods of improved short-time insemination in IVF-ET cycles. Methods From Sep 2009 to Aug 2012,886 fresh cycles of assisted reproductive techniques were analyzed,including 235 cycles of didn't remove cumulus cell (group A), 315 cycles of removed cumulus cell (group B) and 336 cycles of partly removed cumulus cell (group C). The normal fertilization rate, abnormal fertilization rate, cleavage rates, high-quality embryo rate, embryo implantation rate, clinical pregnancy rate and abortion rate were compared. Results There was no significant difference in normal fertilization rate, cleavage rates, embryo implantation rate and abortion rate among the three groups (P〉O.05), but the high-quality embryo rate and clinical pregnancy rate of group C were significantly higher than those in group A and B (P〈O.05). Conclusion Improved short time insemination does not affect fertilization rate, cleavage rates, embryo implantation rate and abortion rate, but can increase high quality embryo rate, clinic pregnancy rate and can reduce the abnormal fertilization rate.
