高频超声对2型糖尿病患者颈动脉功能改变与脉搏速度关系的研究

摘    要:目的应用高频超声研究2型糖尿病患者(T2DM)颈动脉功能改变与脉搏速度(PWV)的变化及其关系。方法选择T2DM患者61例,按病程分为2组:病程≤5 a为A组(30例),5 a为B组(31例);另选健康体检者35例为C组。应用二维超声测量3组双侧颈动脉内-中膜厚度(IMT),M型超声记录颈总动脉(CCA)前后壁的运动幅度变化,测量舒张期和收缩期内径,计算内-中膜横截面(IMSCA)、僵硬度指数、扩张性和顺应性等参数。用彩色多普勒超声及心电图测量计算PWV。结果B组比A组及对照组IMSCA、僵硬度指数增大,PWV加快,扩张性和顺应性降低(P0.05)。IMSCA、僵硬度指数与脉压正相关(P0.01),PWV与僵硬度指数正相关(P0.05)。结论2型糖尿病患者僵硬度指数增大,弹性功能降低,PWV加快。

Abstract:Objective It is to explore the alterations and its relationship of the function and pulse wave velocity(PWV) of carotid in type 2 diabetes(T2DM) patients by high-frequency ultrasound.Methods 61 patients with T2DM were chosen and divided into 2 groups according to their disease course: group A(30 cases) whose course≤5 a and group B(31 cases)whose course >5 a.35 cases of healthy persons were chosen as group C.The intima-media thickness(IMT) of the three groups was measured by two-dimentional ultrasound,M-mode ultrasound was used to record the movement extent change between systolic and diastolic period of anterior and posterior wall of common carotid artery(CCA),the diameter of systolic and diastolic period were measured,the cross-sectional area(IMSCA),stiffness index,distensibility and compliance were calculated.PWV was detected and calculated by color Doppler ultrasound and ECG.Results Compared with group A and control group,group B has a higher IMSCA and stiffness index,faster PWV and lower distensibility and compliance(P<0.05).IMSCA and stiffness index was positively related to pulse pressure(P<0.01).PWV was positively correlated to vascular stiffness index(P<0.05).Conclusion Patients with T2DM have higher stiffness index,worse elasticity and faster PWV.
