不同TNM分期肝癌患者微血管密度与肿瘤标志物水平相关性研究

摘    要:目的 探讨不同TNM分期肝癌患者微血管密度(microvessel density,MVD)与肿瘤标志物水平的相关性.方法 选取我院接受肝癌手术治疗的患者70例,同期因外伤接受肝脏手术的患者20例,健康体检者20例,采用免疫组织化学法检测不同分期肿瘤组织、癌旁组织以及正常肝脏组织中的MVD水平,采用酶联免疫分析法(ELISA)检测血清甲胎蛋白(AFP)、恶性肿瘤特异性生长因子(TSGF)、血管内皮细胞生长因子(VEGF)水平.结果 与正常肝脏组织比较,肿瘤组织及癌旁组织中MVD水平较高,且肿瘤组织高于癌旁组织(P<0.05);肿瘤组织中MVD水平随TNM分期增高而增加,且不同分期间比较差异有统计学意义(P<0.05);与健康体检者比较,肝癌患者血清AFP、TS-GF、VEGF水平明显较高(P<0.05);肝癌患者血清AFP、TSGF、VEGF水平随TNM分期增高而增加,且不同分期比较差异有统计学意义(P<0.05);肝癌患者血清AFP、TSGF、VEGF水平分别与MVD呈正相关(r值分别为0.853,0.920,0.846,P<0.05).结论 肝癌患者肿瘤组织的MVD及肿瘤标志物AFP、TSGF、VEGF水平与临床TNM分期有关,TNM分期越高,MVD及AFP、TSGF、VEGF水平越高,且患者血清AFP、TSGF、VEGF水平分别与MVD呈正相关关系,提示肿瘤标志物AFP、TSGF、VEGF水平与肝癌侵袭、转移能力有关.

Study on the correlation between microvessel density and tumor marker levels in different TNM stages of liver cancer
Abstract:Objective To investigate the correlation between microvessel density(MVD)and tumor marker levels in different TNM stages of liver cancer. Methods Seventy patients with liver cancer who received surgical treatment were enrolled as liver cancer group,the other 20 patients with liver external injury who underwent surgery were enrolled as liver surgery group, moreover 20 healthy subjects were enrolled as control group,The MDA levels were detected by immunohistochemistry in tumor tissues,paracancerous tissues and normal liver tissues,and the serum levels of alpha fetoprotein (AFP),tumor specific growth factor (TSGF)and vascular endothelial growth factor (VEGF)were detected by enzyme linked immunosorbent assay (ELISA). Results As compared with those in normal liver tissues,the levels of MVD in tumor tissues and adjacent tissues were higher,moreover,which in tumor tissues were significantly higher than those in adjacent tissues (P < 0. 05). The levels of MVD in tumor tissues were increased with the increase of TNM stages,furthermore,there were significant differences in the levels of MVD among different stages,stageⅠ < stageⅡ < stageⅢ < stageⅣ (P < 0. 05). As compared with those in control group,the serum levels of AFP,TSGF and VEGF were significantly increased in liver cancer group (P < 0. 05). The serum levels of AFP,TSGF,VEGF were increased with the increase of TNM stages,moreover,there were significant differences among different stages,stageⅠ < stageⅡ < stageⅢ < stageⅣ (P < 0. 05). Besides the serum levels of AFP,TSGF and VEGF were positively correlated with those of MVD in liver cancer group (r = 0. 853,0. 920,0. 846, respectively,P < 0. 05). Conclusion The levels of MVD and tumor markers-AFP,TSGF,VEGF in tumor tissues of patients with liver cancer are correlated to clinical TNM stage,the higher the TNM stage,the higher the levels of MVD,AFP,TSGF, VEGF,moreover,the serum levels of AFP,TSGF,VEGF are positively correlated with those of MVD,it is suggested that the levels of AFP,TSGF,VEGF are related with the invasion and metastasis of liver cancer.
