大型听神经瘤显微外科手术治疗(附60例报告)

目的 总结经枕下乙状窦后入路显微外科手术切除大型听神经瘤的经验和技巧，以提高肿瘤的全切率和面神经的保留率。并发症和降低病死率。方法 对我科近5年来经枕下乙状窦后入路显微手术治疗的60例大型听神经瘤进行回顾性分析研究。结果 本组肿瘤全切率为100%，面神经解剖保留率为78.3%，无死亡。结论 枕下乙状窦后入路治疗大型听神经瘤是一种安全有效的入路。对桥小脑角区解剖的熟悉和熟练的手术技巧是术保留面神经的关键。

Microsurgical treatment to large acoustic neuromas: 60 cases report

Objective To summarize surgical results of large acoustic neuromas (LAN) operated via suboccipital retrosigmoid approach ,and to discuss microneurosurgical techniques and skills in operation procedure. Metholds 60 cases of LAN treated microsurgically in our department in the last 5 years were studied retrospectively. Results Total tumour resection was achieved in 100% of the patients. In 78.3% cases the facial nerve was anatomically preserved. No patient died. Conclusion Suboccipital retrosigmoid approach is a safe and efficient way to deal with LAN. Familiar with cerebellopontine angle anatomy and proficiency in surgical skill are the key points to improve facial nerve preservation rate.
