儿童型系统性红斑狼疮的诊断与治疗

目的 :探讨儿童型系统性红斑狼疮 (SLE)的临床特点及治疗经验。方法 :采取回顾性研究方法分析儿童型系统性红斑狼疮的临床特点和治疗疗效 ,并与中青年组SLE相比较。结果 :儿童型狼疮的首发症状中 ,发热、雷诺氏现象、淋巴结肿大以及肝脾肿大的发生率显著高于中青年组 ,免疫学检查Ads -DNA阳性多见 ,并伴有高丙球蛋白血症 ,发生率明显高于中青年组 (P <0 .0 5 )。治疗根据病情活动和脏器累及决定治疗方案 ,可取得良好效果。结论 :儿童型狼疮发病不典型 ,易误诊。我国 1982年 3月SLE诊断标准较美国风湿病学会标准更有利于儿童型狼疮的早期诊断 ,依疾病活动及脏器累及制订治疗方案 ,可取得良好效果。

TWENTY - THREE CASES OF SYSTEMIC LUPUS ERYTHEMATOSUS IN CHILDREN

Objective:To explore the features of systemic lupus erythematosus (SLE) in children.Methods:Twenty-three cases of SLE in children were reported.Their first sign and clinical manifestation,involvement of organs and laboratory findings were analysed and compared with another 50 cases of SLE in adults who were elected at random.Results:The incidence of fever,vibration white fingers,swelling of lymph nodes,splenomegaly were significantly higher in children patients than that in the adult.Antinuclear antibody (ANA) was often positive,and accompanied with hypergammaglobulinemia in children ( P <0.05).According to patients' condition,determining the therapy,we can gain well effect.Conclusion:The onset of illness in children is not a typical example.The SLE diagnosis norm of Beijing China in 1982 is more advantageous than that of American rheumatology college.We can diagnose it in earlier period in children with SLE,and treat it in time.
