卵巢囊腺瘤和囊腺癌的螺旋CT诊断与鉴别诊断价值

目的 探讨螺旋CT对卵巢囊腺瘤和囊腺癌的鉴别诊断价值.方法 选取本院收治、行择期囊肿剔除术并经术后病理证实的卵巢囊腺瘤患者33例及卵巢囊腺癌患者9例,对其临床资料和术前CT影像特征进行回顾性分析和对照研究.结果 33例良性肿瘤患者包括浆液性23例、黏液性10例,9例恶性肿瘤患者包括浆液性6例、黏液性3例,均以浆液性居多.CT特征分析结果显示,浆液性、黏液性卵巢囊腺瘤患者之间,瘤体边缘、瘤体形态（单房/多房）及投影密度差异有统计学意义（P＜0.05或P＜0.01）;良、恶性卵巢囊腺瘤之间,CT值、瘤体直径、瘤体边缘、瘤体形态（实性/囊性、单房/多房、规则/不规则）及投影密度等指标均差异有统计学意义（P＜0.05或P＜0.01）.结论 卵巢囊腺瘤和囊腺癌的CT特征各具代表性,亦存在一定的重合性,在结合患者临床体征的基础上,对各CT指标进行综合性考察,对卵巢囊腺瘤和囊腺癌的鉴别具有较高价值.

Value of spiral CT in the diagnosis of ovarian cystadenoma and cystadenocarcinoma

Objective To investigate the diagnostic value of spiral CT in ovarian cystadenoma and cystade- noearcinoma. Methods Thirty-three patients with ovarian eystadenoma as well as 9 patients with ovarian cystadeno- carcinoma confirmed by postoperative pathological diagnosis were selected in this study, whose data and preoperative CT imaging features were retrospectively and contrastively analyzed. Results There were 23 cases of serous tumor and 10 cases of mueinous tumor in the 33 cases of cystadenoma patients, and there were 6 cases of serous tumor and 3 cases of mucinous tumor in the 9 cases of cystadenocarcinoma patients, with serous tumor as the majority. CT character- istics analysis showed that there was significant difference in characteristics of tumor edge, tumor morphology （single room/multi rooms） and projection density between patients with serous tumor and mutinous tumor （P〈0.05 or P〈0.01）. Further more, there was also significant difference in characteristics of CT value, tumor diameter, tumor edge and tu- mor morphology （solid/cystic, single room/multi rooms, regular/irregular） and projection density between patients with ovarian eystadenoma and cystadenocarcinoma （P〈0.05 or P〈0.01）. Conclusion CT characteristics of ovarian cystadenoma and cystadenocarcinoma both have specificity and intersectionality, and it has high value to identify them to make comprehensive investigation of multi CT characteristics in combination with clinical signs.
