早期应用肺表面活性物质防治早产儿呼吸窘迫综合征34例效果观察

摘    要:目的:探讨早期应用肺表面活性物质预防早产儿呼吸窘迫综合征的疗效。方法:将68例呼吸窘迫综合征早产儿随机分为两组各34例,观察组于生后24 h内经气管插管内注入肺表面活性物质;对照组于出生24 h后气管插管内注入肺表面活性物质。观察两组患儿肺功能改善情况,对两组患儿住院天数、机械通气天数、用氧天数等方面进行比较。结果:经肺表面活性物质治疗后患儿的动脉氧分压(PaO2)、动脉血氧分压/吸入氧气浓度(PaO2/FiO2)、pH值及动脉二氧化碳分压(PaCO2)与用药前比较明显改善,差异有统计学意义(P<0.05);观察组各项指标与对照组比较改善明显,差异有统计学意义(P<0.05);观察组机械通气时间、用氧时间及住院时间与对照组比较明显缩短,差异有统计学意义(P<0.05)。结论:肺表面活性物质替代治疗能明显改善呼吸窘迫综合征早产儿的肺功能,且早期应用效果更好。

Observation on the effect of early use of pulmonary surfactant in the prevention and treatment of 34 premature infants with respiratory distress syndrome

Abstract:Objective: To investigate the curative effect of early use of pulmonary surfactant in the prevention and treatment of premature infants with respiratory distress syndrome.Methods: 68 premature infants were randomly divided into an observation group and a control group(34 infants in each group).The premature infants in the observation group were given pulmonary surfactant injected via endotracheal intubation within 24 hours after birth and the pulmonary surfactant was given after 24 hours in the control group.The improvement of lung function of the infants was observed and hospitalization days,mechanical ventilation days and the days of using oxygen were compared between the two groups.Results: PaO2,PaO2/FiO2,pH values and PaCO2 were significantly improved after the treatment with pulmonary surfactant compared with those before the treatment,the differences were statistically significant(P<0.05);the comparison of the indicators between the two groups was significantly different(P<0.05);the differences in the comparison of mechanical ventilation time,the time of using oxygen and hospitalization days were statistically significant between the two groups(P<0.05).Conclusion: The pulmonary surfactant replacement therapy can significantly improve the lung function of premature infants with respiratory distress syndrome,moreover the effect will be much better if it is applied earlier.
