C反应蛋白和D-二聚体在肺癌转移诊断中的应用价值

摘    要:目的观察肺癌患者C反应蛋白(CRP)和D-二聚体(D-D)的变化,探讨其对肺癌转移诊断的意义。方法分别检测50例肺癌患者、20例良性肺疾病患者和25例正常对照者血清Hs-CRP、血浆D-D、纤维蛋白原(Fib)浓度,并计算肺癌有无转移患者各指标的阳性百分率。结果肺癌组Hs-CRP、Fib、D-D水平均高于正常对照组和良性肺病组(t分别=12.31、13.12、7.12;6.15、8.31、5.34,P均<0.05);肺癌有转移组Hs-CRP、Fib、D-D水平均高于无转移组(t分别=8.71、7.42、7.01,P均<0.05)。肺癌无转移组及有转移组单项指标Hs-CRP阳性率最高,肺癌有转移组Hs-CRP及D-D阳性率显著高于无转移组(χ2分别=4.34、4.79,P均<0.05),但两组间Fib阳性率差异无统计学意义(χ2=2.14,P>0.05);肺癌有转移组Hs-CRP与D-D组合的阳性率显著高于单项Hs-CRP和D-D的阳性率(χ2分别=3.85、6.12,P均<0.05)。结论联合检测Hs-CRP和D-D可作为肺癌有无转移的警示指标,为临床治疗方案制定提供参考依据。

Clinical significance of C reactive protein and d -dimer in the diagnosis of lung cancer CHEN Huihong, QI

Abstract:Objective To investigate the clinical values of C-reactive protein and d-dimer in the diagnosis of lung cancer. Methods Blood samples were gotten from 50 patients with lung cancer, 20 patients with benign lung diseases and 25 healthy volunteers. The concentration of high-sensitivity C-reactive protein (hs-CRP)and d-dimer (D-D) was, respectively, determined by immune turbidity method. And the concentration of fibrinogen(Fib) was detected simultaneously. Results The levels of hs-CRP, D-D and Fib were significantly higher in the group of lung cancer than those in the groups of benign lung disease and healthy control (t = 12.31,13.12,7.12 ; 6.15,8.31,5.34, P〈0.05). The levels of hs -C RP, D-D and Fib were remarkably higher in the group of lung cancer with metastasis than those in that without metastasis (t= 8.71,7.42,7.01 ,P〈0.05). Compared with other markers, hs-CRP had a highest positive rate in the either group of lung cancer. The positive rates of hs-CRP and D-D in the group of lung cancer with metastasis were markedly higher than those in that without metastasis (χ2=4.34,4.79,P〈0.05), but there was no significant difference for Fid between the two groups(χ2=2.14,P〉0.05).The positive rate of the combination of Hs-CRP and D-D were notably higher than either hs-CRP or D-D in the group of lung cancer with metastasis(χ2=3.85,6.12,P〈0.05). Conclusions The combination of hs-CRP and D-D is useful as indicator of lung cancer metastasis, which can provide valuable reference for lung cancer therapy.
