退变性腰椎滑脱与关节突关节的方向性

目的：探讨腰椎关节突关节的方向性在退性变腰椎滑脱发生中的病因学意义。方法：34例L4／5退变性腰椎滑脱患者及30名正常对照者的CT扫描片，侧位X线片上关节突关节的方向性及腰椎滑脱程度进行分析。结果：退变性腰椎滑脱患者的关节突关节方向与对照组比较更偏向吴矢状位（P＜0．01），关节突关节不对称程度也更为明显（P＜0．05），小关节椎弓根角更倾向于水平位（P＜0．01），关节突关节角，不对称程度及小关节椎弓根角与腰椎滑脱程度无显著相关性（P＞0．05）。结论：腰椎关节突关节的方向性在退变性腰椎滑脱的发生中可能有一定的病因学意义。

Degenerative lumbar spondylolisthesis and orientation of facet joint

Objective:To research the etiologic meaning of the orientation of the lumbar facet joint in the occurrence of degenerative spondylolisthesis.Method:The features of the lumbar facet joint (facet joint angle,facet tropism, degree of spondylolisthesis) of 34 cases of degenerative spondylolisthesis patients and 30 normal subjects were studied using CT scanning and lateral X-ray film.Result:The mean orientation of the lumbar facet angles relative to the coronal plane was more sagittal in the patients who had degenerative spondylolisthesis(55.59 degrees versus 40.48 degrees,P<0.01).The tropism of the facet angle was greater in the patients who had degenerative spondylolisthesis(P<0.01).The facet angle relative to the pedicle was more horizontal in the patients who had degenerative spondylolisthesis(100.33 degrees versus 117.12 degrees,P<0.01).There was no significant correlation between facet angle,facet joint tropism and the grade of degenerative spondylolisthesis. Conclusion:The orientation of lumbar facet joint has an important etiologic meaning in the occurrence of degenerative lumbar spondylolisthesis.
