上海某社区高血压细节管理效果评价

目的评价高血压细节管理模式对高血压控制的效果。方法在江桥社区招募600名高血压患者为研究组，马陆社区195名高血压患者为对照组，研究组采用社区医生和健康管理专员相结合的细节管理模式，开展以“饮食、控烟、规则服药、运动、自我效能”为专题的干预活动，对照组常规高血压管理。结果经过1年干预，研究组收缩压、舒张压、腹围、体重较干预前均有明显下降，差异有统计学意义（P〈0．05）；对照组干预后收缩压有所下降，舒张压、腹围和体重与干预前比较差异无统计学意义（P〉0．05）。结论高血压细节管理模式优于常规管理，能有效改善高血压患者生活质量，值得在社区推广。

Evaluation of the detail management effect of hypertension in a community in Shanghai

Objective Evaluating the effect of the detail management pattern to hypertension management. Method 600 hypertension patients were enrolled in Jiangqiao town for study group. 195 hypertension patients were enrolled as the comparing group. The study group were managed by the community health doctors and health care managers with the interference activities of ＂controlling of diet and smoking, regularly taking drugs, sports, and self management＂. But the comparing group just was treated with regular medicine. Results After one year interference, the systolic and diastolic pressures, the abdomen and weight of the patients were fallen ob- viously. The difference of statistic has significance （P〈0.05）. The comparing group diastolic fell, but the systolic and abdomen and weight has no statistic different significance （P〉0.05）. Conclusion The detail manage- ment for hypertension pattern is superior to regular hypertension treatment, which can enhance the quality of life of the hypertension patients. It is worthy spreading in community.
