髋周骨内腱鞘囊肿的影像学诊断

摘    要:①目的 探讨髋周骨内腱鞘囊肿的X线和CT表现及其诊断价值。②方法 回顾性阅读经病理证实的9例髋周骨内腱鞘囊肿的X线平片和CT片。③结果 9例病人X线平片显示9个病灶,CT显示11个病灶。CT显示病灶长径0.5~2.4cm,边界清楚,有薄层硬化缘。其中6个病灶紧邻股骨头承重区关节面下,1个病灶位于股骨头中心部,4个病灶位于髋臼顶前唇交界处。10个病灶呈圆形和类圆形,1个位于股骨头中心者呈花瓣样多囊状。6个病灶为软组织或液体密度,4个病灶为气体和软组织或液体密度并存,1个病灶为单纯气体密度。④结论 X线检查简单易行,可作为髋周骨内腱鞘囊肿的首选影像学检查方法。CT对病灶有更高的检出率,并可对多数病人做出诊断。

IMAGE DIAGNOSIS OF INTRAOSSEOUS GANGLION CYST SURROUNDING THE HIP JOINT

Abstract:Objective To study the X-ray and CT features of intraosseous ganglion cysts surrounding hip joint and their diagnostic value. Methods The X-ray films and CT images of nine cases of the disease previously confirmed pathologically were reviewed retrospectively. Results Nine foci were showed in the plain films of nine cases, 11 in the CT films, with clear and sclerotic edges. Their long diameter ranged from 0.5 cm to 2.4 cm, six foci overlaid by load-bearing surface of femoral head, one focus located in the center of femoral head, four foci in the connective part of the roof and anterior lip of acetabulum. A round and elliptical shape cyst was seen in 10 foci, one in the center of femoral head manifested as a petal-like polycystic change. Six foci appeared density of soft tissue or liquid, four foci with soft tissue or liquid and air, and one focus with air only. Conclusion X-ray is simple and easy to perform and can be the first of choice of image examination for the disease around the hip. CT is more sensitive for most patients.
