原发性视神经萎缩LED刺激器多焦视网膜电图二阶反应波形研究

摘    要:目的:研究原发性视神经萎缩(PONA)患者二极管发光器(LED)的多焦视网膜电图(mf-ERG)二阶反应波形特点。方法:选取正常人18例18只眼和11例原发于视神经挫伤、球后视神经炎、缺血性视乳头病变导致PO-NA 15眼,采用视觉电生理记录仪(RETI Scan3.15)分别作阴极射线管(CRT)及LED的mf-ERG二阶反应检查。CRT刺激器记录采用国际临床视觉电生理推荐标准记录,LED刺激时间为10/10(16.7 ms)。观察CRT及LED的mf-ERG总和反应波形特点,两种刺激器记录波形分别比较正常组和PONA组mf-ERG二阶反应形态、振幅密度。结果:PONA组CRT刺激器的mf-ERG二阶反应波形N2波振幅比正常组增加,但两组之间没有显著性差异(P>0.05)。而PONA组LED刺激器N2波振幅比正常组减小,两组之间有显著性差异(P<0.05)。结论:LED刺激器mf-ERG二阶反应波形更丰富,其N2波可能包涵了更多视网膜内层信息。

Abstract:Objective:To investigate the waveform of second-order kernel of multifocal electroretinogram(mf-ERG) stimulated by light emitting diode(LED) in primary ocular nerve atrophy(PONA).Methods:Eighteen eyes in 18 normal controls and fifteen eyes in 11 patients of PONA were took into this study.They accepted the multifocal electroretinogram stimulated by cathode ray tube(CRT) and LED using the Roland RETI Scan 3.15 system.The stimulation time of LED was 10/10(16.7 ms).The second-order kernel(SOK) of the summed responses was observed.The waveform,amplitude and implicit times of mf-ERG summed response were analyzed.Results:In the SOK responses of PONA in CRT mf-ERG,the amplitude of N2 waveform were slightly increased than that of normal control without stastical difference(P>0.05).However,the amplitude of N2 waveform of PONA were obviously decreased than that of normal control in LED mf-ERG(P<0.05).Conclusion:The waveform stimulated by LED is more complicated,and there might be more inner retina informations.
