降钙素原和C-反应蛋白在三种呼吸疾病中的变化及临床意义

目的:探讨降钙素原(PCT)和C-反应蛋白(CRP)在社区获得性肺炎(CAP)、慢性阻塞性肺疾病急性加重(AECOPD)及支气管哮喘急性发作中的变化及临床意义。方法:选取31例CAP,34例AECOPD和30例支气管哮喘急性发作患者,分别在治疗前和经治疗后达到临床出院标准时测定血清PCT及CRP。结果:三组患者治疗前PCT及CRP均显著高于治疗后(P<0.05);PCT在CAP组最高,AECOPD组次之,支气管哮喘急性发作最低,差异有统计学意义(P<0.05);CRP在三组中无明显差异(P>0.05);经治疗后三组患者PCT及CRP无明显差异(P>0.05)。结论:在CAP、AECOPD及支气管哮喘急性发作患者中,PCT和CRP是较好的炎性反应指标,尤其在细菌性炎性反应的判断上PCT较CRP可能更有临床价值。

Clinical Value of Serum Procalcitonin And C-reactive protein Changes in Three Kinds of Respiratory Disease

Objective To investigate the clinical value of serum procalcitonin（ PCT） and C-reactive protein（ CRP） changes in patients with community acquired pneumonia（ CAP）,acute exacerbation of chronic obstructive pulmonary disease（ AECOPD） and acute attack of bronchial asthma.Method The level of PCT and CRP were detected pre and post treatment in 31 patients with CAP,34 patients with AECOPD and 30 patients with acute attack of bronchial asthma.Results The level of PCT and CRP in pre-treatment were significantly increased than those in post-treatment among three groups（ P〈0.05）.The level of PCT was higher in CAP group than those in AECOPD group and acute attack of bronchial asthma group,meanwhile AECOPD group was higher than acute attack of bronchial asthma group,the difference was significant（ P〈0.05）.The level of CRP was no difference among three groups（ P〈0.05）.There were no difference in PCT and CRP among three groups（ P〈0.05）.Conclusion The PCT and CRP are sensitive index for inflammatory reaction in patients in patients with CAP,AECOPD and acute attack of bronchial asthma,especially in bacterial infections,PCT maybe has more clinical value.
