摘    要:目的:探讨两种声门下滞留物吸引法在ICU气管切开患者中的应用效果。方法:将入住ICU行气管切开并机械通气的患者104例,按入院序号随机分为A组与B组,A组采用声门下间断吸引、冲洗的方法,B组采用持续声门下吸引加间断冲洗的方法,观察两组2周内呼吸机相关性肺炎(VAP)发生例数,发生时间、机械通气时间及呛咳和吸引管堵管例数。结果:B组2周内VAP发生例数、机械通气时间及呛咳和吸引管堵管例数明显少于A组(P<0.05),B组VAP发生时间明显延缓于A组(P<0.05)。结论:对ICU气管切开行机械通气患者实行声门下持续负压吸引加间断冲洗方法,不仅能够达到清除气囊上方滞留物的目的,而且还能有效的降低VAP的发生。

Abstract:Objective Discuss the application result of two methods that attracted subglottic retentate in ICU patients with tracheotomy. Methods 104 cases of patients with tracheotomy and mechanical ventilation,according to the admission number were randomly divided into group A and group B,Patients in group A were treated by subglottic discontinuous suction and washing,Patients in group B were treated by continuous subglottic attract plus intermittent flushing,Observe the number of cases of ventilator associated pneumonia(VAP) occurred,and time of occurrence,Observe the number of cases of mechanical ventilation time and choking cough and attract pipe plugging,in two group,in two weeks. Results The patients with VAP, mechanical ventilation time and choking cough and attract pipe wall in group B were significantly less than group A(P 〈 0.05),compare with group A,the patient ~;ith VAP had significantly delay time in group B(P 〈 0.05).Conclusion ICU patients with trachea cut operation mechanical ventilation were treated by subglottic continued pressure suction and intermittent flushing,not only can achieve the goal of clear gasbag retentate above,but also can effectively reduce the occurrence of VAP.
