妇科腹腔镜手术严重并发症12例分析

目的 探讨腹腔镜手术严重并发症的相关情况，以预防和减少其发生。方法 采用回顾性分析方法对该院妇科近5年12例腹腔镜手术严重并发症情况进行分析。结果 妇科腹腔镜手术严重并发症发生率为1．53％。腹腔镜下第Ⅳ类手术262例，严重并发症6例；其他类手术523例，严重并发症6例。其中出血4例、盆腔脓肿2例、输尿管损伤2例、肠损伤1例、异位妊娠术后继发腹腔妊娠2例、持续性异位妊娠1例。结论 即使简单的腹腔镜手术也可导致严重并发症。提高医生的内镜手术技能和手术技巧，重视出血、感染及损伤的严重并发症。异位妊娠术后可能继发腹腔妊娠及持续性异位妊娠。

Analysis of 12 cases of serious complications in laparoscopic gynecologic surgery

[Objective] To study the rate, cause and precaution of serious complications in laparoscopic gynecologic surgery. [Method] We analyzed retrospectively the clinical data of 12 cases with serious complications following laparoscopic gynecologic surgery performed from May 1998 to may 2003. [Results] The overall incidence of operative serious complications was 1.53%(12/785). The bleeding 4, celiac abscess 2, ureteral injury 2, intestinal injury 1, villus is cultivated in celiac 2 and persistent ectopic pregnancy 1 after pregnancy of fallopian tube in laparoscopic gynecologic surgery. [Conclusion] In our experience, the complications seems related to all kinds of operations. They are also related to doctor's experiences and skills. Therefore well-training and practice should be emphasized in order to avoid these complications. The villus can be cultivated in celiac after pregnancy of fallopian tube being cuted by laparoscopy, Also it can cause persistent ectopic pregnancy. Doctors should pay attention to the complications of bleeding, infection and injury.
