短期应用糖皮质激素对老年男性慢性阻塞性肺疾病急性加重期患者骨代谢的影响

摘    要:目的观察不同给药方式对短期应用糖皮质激素老年男性慢性阻塞性肺疾病急性加重期(AECOPD)患者骨代谢的影响。方法对74例短期应用糖皮质激素老年男性AECOPD患者(其中口服给药36例,静脉给药38例)及40例健康体检老年男性检测其骨代谢指标水平并测定腰椎(L2~L4)、左侧髋部骨密度(bone mineral density,BMD)。结果①AECOPD患者BMD较正常对照组明显降低,短期应用糖皮质激素后无明显差异变化。②短期应用糖皮质激素后,骨吸收指标抗酒石酸盐酸性磷酸酶(TRAP)明显升高(P〈0.05),而骨形成指标均较前降低,其中静脉给药组胰岛素样生长因子-I(IGF-I),降钙素(CT),甲状旁腺素(PTH)较治疗前明显降低(P〈0.05)。结论慢性阻塞性肺疾病(COPD)患者可引起骨密度明显降低,短期静脉使用糖皮质激素较口服使用更易引起骨代谢指标紊乱。
